Patient question

What happens if I leave my knee arthritis untreated?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

Untreated knee arthritis does not always get rapidly worse, but in many people pain, stiffness and leg deformity slowly increase, the thigh muscles weaken and activity drops. Late presentation can also narrow the options, so a knee that once suited a smaller operation may need a full replacement, warns Dr. Pablo Gelber at Aman Hospital in Doha.

Arthritis progresses at its own pace

Osteoarthritis behaves differently from person to person. Some knees stay stable for many years, while others worsen over a few. Treatment does not reverse cartilage loss, but it reduces pain, keeps the knee moving and often slows the overall decline. Leaving it alone means losing those benefits.

The main risks of doing nothing

  • Increasing stiffness, with loss of the ability to fully straighten or bend the knee
  • Worsening bow-leg or knock-knee deformity as one side collapses
  • Thigh muscle wasting, which makes stairs, rising from a chair and prayer harder
  • More falls in older patients because of weakness and poor balance
  • Weight gain and reduced fitness from inactivity, with effects on heart health and blood sugar
  • Poor sleep and low mood from chronic pain
  • Overloading the other knee, hip or back by limping

Why timing affects surgical options

If surgery is eventually needed, a knee treated earlier often has more choices. Realignment osteotomy and partial knee replacement suit knees with arthritis on one side, good movement and mild deformity. Once arthritis spreads, the knee stiffens badly or the deformity becomes severe, a total replacement may be the only option, and a very stiff knee can recover movement more slowly after surgery.

This is not a reason to rush into an operation, but it is a good reason not to ignore a steadily worsening knee.

Signs it is time for a review in Doha

Book an assessment if pain wakes you at night, you are walking shorter distances each month, the leg is visibly bending more, or simple measures no longer help. Seek urgent care for a hot, red, swollen knee with fever. At Aman Hospital, Dr. Gelber compares your examination with standing X-rays and explains which options are open to you now.

Frequently asked questions

Can untreated knee arthritis become an emergency?

Rarely. Osteoarthritis itself is not an emergency, but a sudden hot, swollen knee with fever may be an infection or gout and needs same-day care.

Is it dangerous to take pain tablets for years instead?

Long-term anti-inflammatory tablets can affect the stomach, kidneys and heart, especially in older people. Use them under medical guidance and combine them with exercise and weight management.
