Patient question

Can I play padel, football or run after a high tibial osteotomy?

Quick answer

Many active people return to sport after a high tibial osteotomy, including padel, cycling, gym work and, for some, running or recreational football. Return usually happens between six and twelve months, once the bone has healed and strength is restored, explains Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha.

Why osteotomy suits active people

Because an osteotomy keeps your own cartilage, meniscus and ligaments, there is no artificial joint surface to protect from impact. This is the main reason it is offered to younger, sporty patients with inner knee wear. Studies show a large proportion of patients return to some sport, although not always at the same intensity as before their knee problems started.

A typical stepwise return

  • Static bike and pool work: often from around six to eight weeks, once the wound has healed
  • Gym strength training without impact: built up from about two to three months
  • Straight-line jogging: usually from around four to six months if X-rays show good healing
  • Padel, tennis and golf: commonly from around six months, starting with gentle doubles or practice sessions
  • Football, basketball and other cutting sports: often from nine to twelve months

Each step depends on bone healing on X-ray, quadriceps strength, a calm knee without swelling and the surgeon's advice.

What affects how far you get back

The amount of cartilage wear before surgery matters most. A knee with moderate wear usually allows more than one with bone-on-bone changes. Other factors include associated procedures such as meniscal or cartilage repair, your weight, your commitment to rehabilitation, and how long you had stopped playing before surgery. Some people choose to swap high-impact sports for lower-impact ones to protect the knee long term.

Training safely in Qatar

Padel is hugely popular in Doha, and the quick stops and lunges load the knee heavily. Start back with short sessions, warm up well, wear court-specific shoes and avoid playing through swelling. In the summer, train indoors or in the cooler early morning and evening, and hydrate well, since fatigue in the heat increases the risk of awkward movements.

Frequently asked questions

Will the plate stop me playing contact sport?

Once the bone has healed, the plate does not usually restrict sport. If it is prominent and gets knocked, removal can be discussed.

My knee swells after padel months later. Is that normal?

Occasional mild swelling after a hard session can happen. Persistent or increasing swelling should be reviewed by your surgeon.
