Patient question

When can I return to football, padel or the gym after PCL reconstruction?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

Most people return to running around six to nine months after PCL reconstruction and to football, padel or other contact and pivoting sport after about nine to twelve months, once quadriceps strength and functional tests are close to the other leg. Combined reconstructions often take longer, explains Dr. Pablo Gelber at Aman Hospital in Doha.

A staged return

  • Cycling and swimming with a straight-leg kick usually come first, once bending allows
  • Gym work starts with quadriceps-focused exercises, adding hamstring loading later and gradually
  • Straight-line running is introduced when strength and control are good and the knee does not swell
  • Agility, cutting and landing drills follow, building towards sport-specific training
  • Full training and then matches come last, after testing

The tests that matter

Dr. Gelber and the physiotherapist usually look for a knee that does not swell after training, full or near-full movement, measured thigh strength close to the uninjured side, good hop and landing quality, and stable results on examination. Confidence matters too. Many athletes find the last stage is psychological as much as physical.

Deep squats, kneeling and sujood

Deep knee bending places the most load on the PCL graft, so heavy squats below parallel, deep lunges and prolonged kneeling are reintroduced last. In the gym that often means partial-range squats and leg presses first. Kneeling for prayer usually returns gradually over the first year, and some people find a thicker prayer mat more comfortable for longer.

Training in Qatar

Football and padel are very popular here, and summer heat makes fatigue come on quicker, which is when landing and turning technique slips. Train indoors or in the cooler evening hours, stay hydrated and build up match time gradually. Keep a maintenance strength programme going long after you return, because quadriceps strength remains the main protector of a PCL-reconstructed knee.

Frequently asked questions

Will I play at the same level again?

Many people do return to their sport, but results vary, especially after combined ligament injuries. Dr. Gelber discusses realistic goals for your knee before and during rehabilitation.

Should I wear a brace when I go back to sport?

It is not always required. Some athletes use a functional brace for the first season for confidence, but it does not replace strength and control.
