Patient question

How long is recovery after PCL reconstruction, and why the special brace?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

Recovery after PCL reconstruction usually takes nine to twelve months for sport, with a dedicated brace holding the shin forward for the first few months and early rehab that protects the new graft from backward sag. Desk work often resumes within weeks, explains Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha, Qatar.

Why PCL rehab is different from ACL rehab

After surgery, gravity and the hamstring muscles both tend to pull the shin backwards, stretching the new graft before it has healed. PCL rehabilitation is designed to prevent that. A dynamic PCL brace pushes gently on the back of the upper shin, isolated hamstring exercises are avoided early on, and bending exercises are often done lying on your front or with the shin supported so that it cannot sag.

Usual stages

  • First weeks: crutches, brace on, controlling swelling, regaining full straightening and quadriceps activation
  • Around 6 to 12 weeks: gradual increase in bending and weight-bearing, walking without crutches as control returns
  • Around 3 to 6 months: progressive strengthening, cycling, balance work, and weaning off the brace when advised
  • Around 6 to 9 months: running, agility and sport-specific drills
  • Usually 9 to 12 months or longer: return to contact or pivoting sport after passing strength and function tests

Combined ligament reconstructions often progress more slowly.

Work, driving and daily life

Desk work can often resume within a few weeks if you can keep the leg elevated. Physical, site and industrial jobs usually need several months or modified duties. Driving depends on which knee was operated on and when you can control the car safely, and is discussed at follow-up. Prayer is often done on a chair in the first months, with kneeling reintroduced later.

Signs to report early

Contact the team promptly if you notice increasing calf pain or swelling, fever, redness or discharge from the wounds, numbness in the foot, or a sudden feeling that the knee has slipped backwards. Regular reviews let Dr. Gelber check that the knee is not sagging and adjust the programme.

Frequently asked questions

Do I sleep in the PCL brace?

Often yes in the early period, because the shin can sag when the leg relaxes. Your surgeon will tell you when it is safe to remove it at night.

When can I do hamstring exercises again?

Usually after the first few months, introduced gradually by the physiotherapist once the graft has had time to heal.
