Patient question
After ACL reconstruction, how do I avoid tearing the graft or my other knee?
Quick answer
After ACL reconstruction, both the graft and the uninjured knee carry a higher risk of tearing than in people who never had an ACL injury. Meeting strength and hop-test targets before returning, waiting long enough, and continuing prevention training lower that risk, explains Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha.
Why the other knee is at risk too
The same factors that contributed to the first tear, such as movement patterns, anatomy and sport, usually apply to both legs. During rehabilitation, people also tend to load the healthy leg more, and it can become the stronger, relied-on side in sudden movements. Both knees deserve attention.
What lowers the risk
- Waiting until criteria are met, commonly around nine months or more for pivoting sport
- Thigh strength in the operated leg close to that of the other leg on testing
- Passing hop and agility tests with good technique, not just distance
- Psychological readiness: feeling confident, not fearful, when cutting
- A gradual return: training first, then partial matches, then full competition
- Continuing a prevention warm-up two or three times a week for as long as you play
Sport-specific points in Qatar
Football on artificial turf and padel on sand-filled courts both involve sharp stops and turns. Match fitness takes time, and fatigue late in a game raises risk, so build up the number of games per week gradually. In the summer, playing at cooler times or indoors reduces heat fatigue. Some surgeons add an extra lateral procedure at reconstruction in selected high-risk patients to improve rotational control; this is discussed before surgery.
Staying on track
Keep strength work in your routine long after formal physiotherapy ends. If either knee gives way, swells, or feels unstable, stop playing and get it checked. Dr. Gelber reviews return-to-sport readiness at follow-up at Aman Hospital in Doha.
Frequently asked questions
Should I wear a brace on the other knee?
Braces are not a reliable way to prevent ACL tears. Training strength, landing and cutting technique is more effective.
Does a different sport reduce the risk?
Sports without pivoting, such as cycling, swimming and running, carry much less ACL risk. Some people choose them; others return to their sport with careful preparation.
