Patient question
Does an osteotomy make a future knee replacement harder?
Quick answer
A previous osteotomy can add some technical steps to a later knee replacement, such as removing the plate, but in experienced hands a replacement is still generally possible with good results. Careful planning of the original osteotomy helps keep that door open, explains Dr. Pablo Gelber, knee surgeon in Doha, Qatar.
Myth: an osteotomy rules out a replacement later
It does not. Many patients who have an osteotomy never need a replacement, and those who do can usually have one. The surgeon may need to remove the plate and screws, account for changes in bone shape and pay attention to old scars, but these are familiar challenges. Planning the osteotomy so that alignment is not overcorrected also makes any future replacement simpler.
Myth: it just delays the inevitable
For the right patient, an osteotomy is a joint-preserving operation that keeps your own knee, including its ligaments and natural feel, and can allow high activity levels. If it buys many years of active life, especially for someone in their forties or fifties, that is a meaningful benefit in itself, not simply a delay. Whether a replacement is ever needed varies between individuals.
Myth: osteotomy is old-fashioned
Osteotomy has been performed for decades, but planning and fixation have improved considerably, with precise alignment measurement on long-leg X-rays and stable locking plates. It remains a recognised option for selected patients with bow legs and wear on the inner side of the knee.
Myth: once the bone heals, the plate must come out
Not necessarily. Many people keep the plate without problems. It may be removed if it irritates the soft tissues, if you prefer, or at the time of a later replacement. This is discussed at follow-up once the bone has fully healed.
Making the choice in Doha
At a consultation at Aman Hospital, Dr. Gelber looks at your alignment, the state of the rest of the knee, your age and your activity goals, and explains honestly whether osteotomy, partial replacement or total replacement fits you better.
Frequently asked questions
How long does an osteotomy usually last?
It varies widely with patient selection, weight, activity and the degree of arthritis at the time of surgery. Your surgeon can give a realistic expectation for your knee after examination.
Is the recovery from replacement after osteotomy longer?
Usually it is similar to a standard replacement, although the operation itself can take longer if hardware must be removed.
