Patient question

Can I return to football after revision ACL surgery?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

Many people do return to football after revision ACL surgery, usually no earlier than nine to twelve months and only after passing strength, hop and movement tests. Whether returning to the same level is wise depends on the knee's cartilage, meniscus and history, says Doha knee surgeon Dr. Pablo Gelber at Aman Hospital.

Timing is set by tests, not dates

Football, padel and basketball all involve cutting, landing and sudden stops, which load the new graft heavily. Before a return to these sports, Dr. Gelber and the physiotherapist usually look for:

  • A knee that is fully straight, bends well and does not swell after training
  • Thigh strength close to the other leg, measured rather than guessed
  • Good results on single-leg hop and landing tests
  • Confidence in the knee during sport-specific drills
  • Completion of a structured on-field programme before full matches

Deciding on the level of sport

After a second reconstruction, some people return to competitive football, others choose five-a-side with friends, and some move to sports with less pivoting. The decision depends on the state of the cartilage and meniscus, the number of previous injuries, age and how important the sport is in your life.

There is no single correct answer. The aim is a decision you make with clear information, rather than one made by pressure from a team or a season.

Protecting the knee once you are back

Injury-prevention warm-ups that train landing, cutting and hamstring strength lower the risk of a new injury and should continue long after rehabilitation ends. In Qatar, training in the cooler evening hours or indoors during summer reduces fatigue, which is when technique tends to slip. Good footwear for the surface, whether grass, artificial turf or a padel court, also helps.

When to stop and get checked

Stop and book a review if the knee swells after matches, gives way, or develops pain on the inner or outer joint line. These can be early signs of meniscus or cartilage trouble that are easier to manage when caught early.

Frequently asked questions

Will I need a knee brace for football?

A brace is not usually required once the tests are passed. Some players like one for confidence early on, but it does not replace strength and control.

Is padel safer than football after revision?

Padel still involves quick turns and lunges, so it also needs a graded return. It is often a little less demanding than eleven-a-side football, but the same tests apply.
