Patient question
Why is my knee still painful months after a knee replacement?
Quick answer
Some pain and aching can take up to a year to settle after a knee replacement, especially with stairs and kneeling. Pain that is getting worse, or still severe beyond three to six months, needs investigation for causes such as stiffness, infection, loosening, instability, kneecap problems or pain coming from the hip or back, says Dr. Pablo Gelber in Doha.
What normal recovery looks like
Most patients improve steadily over the first three months, then more slowly up to a year. Aching after a busy day, mild swelling and discomfort when kneeling are common during this time. Recovery is rarely a straight line, and a hard physiotherapy week can bring a temporary setback. Persistent pain should be judged against this expected curve.
Causes inside the knee
- Stiffness from scar tissue, when the knee will not bend or straighten enough
- Infection, sometimes low-grade without fever, especially if pain never settled after surgery
- Loosening of a component from the bone
- Instability, when the ligaments are too loose and the knee feels wobbly
- Kneecap problems, such as tracking or pain behind the kneecap
- Irritation of soft tissue, a tendon or the joint lining
Causes outside the knee
Pain can be referred to the knee from an arthritic hip or a trapped nerve in the lower back. Nerve sensitivity around the scar, complex regional pain syndrome and, occasionally, a vascular problem can also cause persistent symptoms. A careful examination of the hip and spine is part of any assessment of a painful knee replacement.
How it is investigated
Assessment starts with your history and examination, then X-rays to check the position and fixation of the implant. Blood tests for inflammation help screen for infection, and sometimes fluid is drawn from the knee for analysis. CT scans or bone scans are used for specific questions. The aim is to find a cause before considering any further surgery.
Getting it checked in Doha
If your replaced knee is still painful, whether it was done in Qatar or abroad, Dr. Gelber can review it at Aman Hospital. Bring your operation notes and previous X-rays if you have them. Many causes are treatable without another operation, and when a revision is needed, it is planned around the specific problem.
Frequently asked questions
Is more physiotherapy the answer?
Sometimes, particularly for weakness or mild stiffness. But if pain is worsening or there are signs of infection or loosening, more exercise alone will not solve it and investigation comes first.
Does persistent pain always mean revision surgery?
No. Many patients improve with targeted treatment. Revision is considered only when a clear mechanical or infective cause is found.
