Patient question
When can I run and play sport again after cartilage repair?
Quick answer
Light running is usually considered from around six to nine months after cartilage restoration, and pivoting sports such as football and padel from around nine to twelve months or later. Progress is guided by milestones rather than the calendar alone, explains Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha.
Why cartilage takes longer
Repair tissue and grafts keep maturing for many months after the knee feels comfortable. Running and cutting place high, repeated loads across the repaired surface, so returning too early risks damaging tissue that is not yet ready. This is why cartilage surgery has a longer return than, for example, a meniscus trim.
Milestones that usually come first
- No swelling after daily activity and exercise
- Full or near-full range of motion
- Thigh strength close to the other leg on testing
- Good balance and single-leg control
- Comfortable on the bike, cross-trainer and in the pool
- Pain-controlled hopping and jogging drills in the clinic
Your physiotherapist will usually progress from straight-line jogging to change of direction, then sport-specific drills, then training, before matches.
What changes the timeline
The technique matters: marrow stimulation, osteochondral grafts and cell-based repairs mature at different rates. Defect size and location, combined procedures such as ACL reconstruction or osteotomy, and your age and conditioning all play a part. Some patients return to running but choose lower-impact sport long term.
Training safely in Qatar
Plan the return so the hardest phase falls in the cooler months, or train indoors and early in the morning in summer. Hydrate well, as fatigue in the heat makes landing mechanics worse. Dr. Gelber reviews your progress at Aman Hospital and agrees each step of the return with you and your physiotherapist.
Frequently asked questions
Will I be able to play at the same level?
Many patients return to sport, but not all reach their previous level. Outcomes are generally more favourable with a single defect in a younger, stable and well-aligned knee.
Is swimming allowed earlier?
Swimming with a gentle kick is often allowed once the wounds have healed, typically within the first two months. Breaststroke kick is usually avoided until later.
