Patient question

What do grade 1, 2 and 3 MCL tears mean?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

The grade describes how much of the MCL is damaged: grade 1 is a stretch with intact fibres, grade 2 is a partial tear with some looseness, and grade 3 is a complete tear. Most grades heal with bracing and rehabilitation; surgery is mainly for complete tears with other ligament damage, explains Dr. Pablo Gelber in Doha.

The three grades in plain words

  • Grade 1: the ligament is stretched and a few fibres are injured. The knee is sore and tender on the inside but stable when tested.
  • Grade 2: part of the ligament is torn. There is more pain and swelling, and the knee opens slightly on the inside when the doctor applies gentle pressure, but there is a firm end point.
  • Grade 3: the ligament is torn through. The knee opens clearly on the inside under stress, often with less pain than you might expect, because the fibres that send pain signals are torn too.

How the grade is decided

Grading is mainly clinical. The surgeon bends the knee slightly, about 30 degrees, and gently pushes it inward to see how far the inner side opens, then repeats the test with the knee straight. Opening with the knee fully straight suggests that structures beyond the MCL, such as a cruciate ligament or the back of the capsule, are also injured.

An MRI confirms the grade, shows where along the ligament the tear sits and reveals any injury to the ACL, meniscus or bone bruising. The location matters: some tears at the shin bone end heal less reliably than tears near the thigh bone.

What the grade means for treatment

Grades 1 and 2 are treated without surgery in almost all patients, with a period of protection, a brace for grade 2 and a structured physiotherapy programme. Many isolated grade 3 tears also heal well in a hinged brace.

Surgery becomes more likely when a grade 3 tear is combined with an ACL or PCL tear, when the torn end has flipped out of position at the shin bone, or when the knee remains loose on the inside after a proper period of bracing.

Reading your own report

Radiology reports often use words such as sprain, partial-thickness tear, high-grade partial tear or full-thickness tear. These do not always match the clinical grade exactly, so bring the images and the report to your appointment. At Aman Hospital in Doha, Dr. Gelber correlates the scan with the examination before recommending a plan.

Frequently asked questions

Is a grade 3 MCL tear always an operation?

No. An isolated grade 3 tear often heals in a hinged brace. The decision depends on what else is injured, where the tear is and how stable the knee becomes over the first weeks.

Why does my grade 3 tear hurt less than my friend's grade 2?

Pain does not track severity well. In a complete tear the pain fibres can be disrupted, so the amount of pain is not a reliable guide to the grade.
