Patient question

How long does an MCL tear take to heal?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

As a rough guide, a grade 1 MCL sprain settles in about one to three weeks, a grade 2 tear in about three to six weeks, and an isolated grade 3 tear in about two to three months. Healing depends on the exact injury and how rehabilitation goes, notes Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha.

The phases of recovery

Early phase, roughly the first two weeks: pain and swelling settle. Walking, sometimes with crutches, and gentle movement are encouraged. Grade 2 and 3 tears are usually protected in a hinged brace.

Middle phase: strength returns. Physiotherapy focuses on the quadriceps, hamstrings and hip muscles, balance and full range of motion. Cycling on a static bike is often one of the first comfortable exercises.

Late phase: return to running, then to sport-specific drills such as cutting and kicking, once the knee is stable on examination and strength is close to the other side.

What can slow healing down

  • Returning to sport too early and re-injuring the ligament
  • A tear at the shin bone end, which can heal less predictably
  • Associated injuries to the ACL, meniscus or cartilage
  • Long periods of complete immobilisation, which cause stiffness
  • Skipping rehabilitation once the pain fades

Signs your recovery is on track

Each week the knee should feel a little less sore, bend further and tolerate more walking. Tenderness over the ligament can linger for a while after the knee is stable, and a small firm lump on the inner thigh bone sometimes forms where the ligament healed.

If the knee still feels loose on the inside, swells after activity or catches, it should be re-examined rather than pushed through. Persistent instability is one of the situations in which surgery may be reconsidered.

Planning around life in Qatar

Many patients in Doha time their rehab around work and the heat, doing pool sessions or indoor gym work in summer. If you are planning a long flight home during recovery, discuss it in clinic, as a knee in a brace after injury may need advice on movement and clot prevention during travel.

Frequently asked questions

Can I drive with an MCL tear?

With a left knee injury in an automatic car, many people drive once they can walk comfortably. With a right knee injury you need full control of the pedals and should not drive in a brace that limits movement or while taking sedating painkillers.

Why does it still ache after three months?

Mild aching over a healed MCL can persist for some months. Ongoing pain with swelling or catching suggests another problem, such as a meniscus tear, and deserves review.
