Patient question

Should I wear a hinged knee brace for an MCL tear, and for how long?

Quick answer

A hinged brace is commonly used for grade 2 and grade 3 MCL tears because it protects the inner side from sideways stress while still allowing the knee to bend. It is usually worn for several weeks, with physiotherapy alongside, explains Dr. Pablo Gelber, who treats ligament injuries at Aman Hospital in Doha.

What the brace actually does

A hinged brace has rigid side supports and hinges at the knee. It limits the inward bending force that stretches a healing MCL, while letting the knee flex and extend so it does not become stiff. A soft sleeve or elastic support feels comforting but does not provide this side-to-side protection.

For a mild grade 1 sprain, a brace is often unnecessary. Most patients do well with relative rest, ice and early exercises.

How long it is usually worn

Typical ranges are around three to six weeks for a grade 2 tear and around six weeks or a little longer for an isolated grade 3 tear. The surgeon may lock or limit the hinge for a short period at first, then open it up as healing allows. Whether to wear it at night, in the shower or at the gym is decided individually.

Coming out of the brace is guided by examination, not just by the calendar. If the inner side is still opening when tested, protection may be extended.

The exercises that go with it

  • Quadriceps tightening and straight-leg raises from the first days
  • Heel slides and gentle bending to restore movement
  • Static cycling once comfortable
  • Hip and core strengthening
  • Balance work, progressing to single-leg exercises

A brace protects the ligament, but it is the rehabilitation that restores a strong, confident knee.

Getting the right brace in Doha

Fit matters. A brace that slips down puts the hinge in the wrong place and loses its effect. At Aman Hospital, the brace is fitted and adjusted after examination, and the plan is reviewed at follow-up. In summer heat, a thin cotton layer under the straps helps with sweating and skin irritation.

Frequently asked questions

Will I need a brace for sport afterwards?

Some athletes use a functional brace for a period after returning to contact sport. It is optional for most, and strength and control matter more.

Can I pray or sit on the floor while wearing the brace?

The hinge limits deep bending, so kneeling and floor sitting are usually difficult in a brace. Many patients pray seated on a chair for a few weeks until the surgeon allows deeper flexion.
