Patient question
Does an MCL tear need surgery?
Quick answer
Usually not. The MCL has a good blood supply, and most tears, including many complete ones, heal with a hinged brace and physiotherapy. Surgery is considered when the MCL tear comes with ACL or other ligament damage, when the torn end is trapped out of place, or when the knee stays loose, says Dr. Pablo Gelber, knee surgeon in Doha.
Why the MCL heals better than other ligaments
Unlike the ACL, which sits inside the joint fluid and rarely heals once torn, the MCL lies outside the joint, wrapped in well-vascularised tissue. Torn ends usually stay close together, and with the right protection the body lays down scar tissue that restores stability over several weeks. That is the main reason non-surgical treatment is the standard starting point.
Situations where surgery is discussed
- A complete MCL tear combined with an ACL or PCL tear, where the inner side may be repaired or reconstructed at the same time as the cruciate ligament
- A tear at the shin bone end where the ligament has flipped above a tendon and cannot heal back to bone
- A large piece of bone pulled off with the ligament
- Persistent inner-side looseness after a proper period of bracing and rehabilitation
- Knees with several injured ligaments after a high-energy injury
Repair or reconstruction
In the acute phase, usually within the first few weeks, the torn ligament can sometimes be stitched or anchored back to bone, often reinforced with a strong suture tape. When the tissue is poor or the injury is old, the ligament is reconstructed using a tendon graft. The choice depends on timing, tissue quality and the other injuries present.
When the ACL is also torn, many surgeons first allow the MCL to heal in a brace for several weeks and then reconstruct the ACL, while others address both together. This is individualised after examination.
How the decision is made at Aman Hospital
Dr. Gelber examines the knee, reviews the MRI and often re-examines after a few weeks of bracing, because the knee's stability at that point is one of the clearest guides. Your sport, work and goals matter too. A padel player, a footballer and someone with a desk job may have different priorities, and the plan reflects that.
Frequently asked questions
If I delay surgery, will it be too late?
For most isolated MCL tears, a trial of bracing does not close the door on surgery later. In complex multi-ligament injuries, however, early assessment matters because acute repair is sometimes only possible in the first weeks.
Is MCL surgery done by keyhole?
The inner side of the knee is reached through an open incision, often combined with arthroscopy to treat the inside of the joint, such as the ACL or meniscus.
