Patient question

I've been told I need another knee operation. Should I get a second opinion?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

Yes, a second opinion is often worthwhile before a repeat knee operation, because revision surgery is usually more complex than the first. Understanding why the first operation did not succeed shapes what should be done next, explains Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha, Qatar.

Why repeat operations deserve extra thought

A second operation on the same knee has to deal with previous tunnels, scar tissue, implants and sometimes bone loss. The reason the first surgery fell short may also be different from the original problem. For example, an ACL graft may fail because of a new injury, tunnel position, untreated alignment or another ligament that was also injured. Each needs a different solution.

Common situations

  • A re-torn ACL graft or a knee that still feels unstable after reconstruction
  • A meniscus repair that did not heal, with recurring pain or locking
  • Persistent stiffness after surgery
  • Ongoing pain after arthroscopy or a knee replacement
  • Recurrent kneecap dislocation after stabilisation surgery

What the review looks at

Dr. Gelber reviews your operation notes, previous and current imaging, and asks about your recovery and rehabilitation. Examination checks stability, alignment, movement and other ligaments. Investigations may include CT to assess tunnels or bone, long-leg X-rays for alignment, MRI, and blood tests if infection is a possibility. Sometimes revision is done in two stages, for example bone grafting first and reconstruction later.

Sometimes the answer is not surgery

Some problems after surgery improve with focused rehabilitation, time or treatment of a different cause, such as hip weakness or nerve irritation. A second opinion can help confirm whether another operation is likely to help, or whether a non-surgical route deserves a proper trial first.

What to bring

Bring operation notes, discharge summaries, all imaging on disc or digital link with reports, and any physiotherapy records. These make a revision assessment at Aman Hospital much more accurate.

Frequently asked questions

Are the results of revision ACL surgery the same as the first?

Results after revision are generally good but often somewhat less predictable than after a first reconstruction. Your surgeon will discuss realistic goals for your knee.

Can the same surgeon do the revision?

Yes, if you are comfortable with that. A second opinion simply helps you make an informed choice about the plan and who carries it out.
