Patient question

Will walking or exercise wear out my arthritic knee faster?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

No. For most people with knee arthritis, regular walking and strengthening exercise ease pain and improve function rather than speeding up wear. Cartilage depends on movement for its nourishment, and strong muscles protect the joint. Rest makes the knee stiffer and weaker, says Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha, Qatar.

Myth: an arthritic knee should be rested

Cartilage has no blood supply of its own. It is nourished by joint fluid that moves in and out as the knee is loaded and unloaded. Long periods of inactivity starve it, weaken the thigh muscles and make the joint stiffer. Most guidelines put exercise at the centre of arthritis treatment for this reason.

Short rest during a painful flare is reasonable. Stopping all activity for months is not.

Myth: pain during exercise means damage

Mild aching during or after activity is common and is not a sign of harm. A useful rule is that pain up to a moderate level that settles within a day is acceptable. If the knee swells, the pain lasts into the next day or you start limping, reduce the amount and build back more slowly rather than stopping completely.

Myth: running always causes arthritis

Studies of recreational runners have not shown more knee arthritis than in non-runners, and some suggest less. The picture is different when a knee already has advanced arthritis, a damaged meniscus or instability. In those knees, low-impact options such as cycling, swimming, the cross-trainer and brisk walking usually keep you fit with fewer flares.

Other common beliefs

  • Cold drinks or air conditioning do not cause arthritis, although some people feel stiffer in the cold
  • X-ray severity does not always match pain; some knees look worn but function well
  • Knee braces and insoles help some patients but are not a substitute for strengthening
  • Arthritis is not purely an old person's disease and can start in the forties, especially after injury

Getting the right plan in Doha

If you are unsure how much activity your knee can take, an assessment at Aman Hospital helps. Dr. Gelber examines the knee, reviews standing X-rays and can arrange physiotherapy so you have a clear, safe programme.

Frequently asked questions

How much walking is reasonable with knee arthritis?

It varies, but many people do well building up gradually to 30 minutes on most days, split into shorter walks if needed. Supportive shoes and flat ground help.

Are stairs bad for an arthritic knee?

Stairs load the kneecap joint heavily and often hurt, but they are not forbidden. Use the rail, lead up with the better leg and down with the sorer leg, and strengthen the thigh muscles.
