Patient question

Can a meniscal root tear be treated without surgery?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

Non-surgical care can control symptoms well, especially when arthritis is already established, but it does not reattach the root. In Doha, Dr. Pablo Gelber usually recommends it when repair is unlikely to protect the knee, and discusses repair first when the cartilage is still healthy.

What non-surgical treatment can and cannot do

The root rarely heals back onto bone by itself once it has fully torn. Non-surgical care therefore aims to calm pain and swelling, keep the knee strong and reduce load on the inner compartment. It manages the knee as it is rather than restoring the meniscus. For many patients, that is a perfectly reasonable choice.

Who it tends to suit

  • Knees with moderate or advanced arthritis, where repair is unlikely to change the course
  • Partial root tears or tears found incidentally with few symptoms
  • People for whom a long period on crutches is not practical or safe
  • Patients with significant medical conditions that raise surgical risk

What it involves

  • A physiotherapy programme focused on quadriceps, hamstring and hip strength, and balance
  • Activity changes: fewer deep squats and stairs for a while, walking on flat ground, swimming or cycling
  • Weight management, which reduces load on the inner knee
  • An unloader brace in some bow-legged knees
  • Simple pain relief, and in selected cases an injection for a flare
  • A short period with a stick or crutches if there is a bone stress reaction under the cartilage

In Qatar's summer, indoor pools and air-conditioned gyms make it easier to keep exercising without overheating.

Signs it is not working

Review is sensible if pain at night or on standing worsens, swelling keeps returning, you are limping after several weeks of good effort, or a new sudden pain appears, which can signal a stress fracture beneath the cartilage. Dr. Gelber monitors patients on non-surgical plans at Aman Hospital in Doha and changes course if the knee is not responding.

Frequently asked questions

If I try physiotherapy first, will I lose the chance of repair?

Possibly, if you wait many months, because the meniscus can extrude and cartilage can wear. If repair is a realistic option, it should be discussed early rather than after a long trial.

Can PRP or other injections heal a root tear?

There is no good evidence that injections reattach a torn root. They may be used for symptom relief in some knees, but not as a substitute for repair.
