Patient question
Which knee brace should I wear after ACL surgery, and for how long?
Quick answer
Many people do not need a brace after a straightforward ACL reconstruction. A hinged brace is often used for a few weeks if the meniscus or another ligament was repaired, and a sports brace is optional on return to play, explains Doha knee surgeon Dr. Pablo Gelber at Aman Hospital.
The early post-operative brace
A hinged brace has adjustable stops that limit how far the knee bends or straightens. After ACL surgery it may be used when:
- A meniscus repair needs protection from deep bending under load
- The MCL, posterolateral corner or another ligament was also treated
- Cartilage surgery was done at the same time
- Your surgeon wants the knee locked straight while you walk in the first days
When the ACL alone was reconstructed, studies have not shown that routine bracing improves the final result, and many surgeons do not use one.
The functional brace for sport
Some players use a custom or off-the-shelf functional brace when returning to football, padel or basketball. It may help confidence, but evidence that it prevents re-injury is limited. It is never a substitute for strength, hop testing and a proper return-to-sport programme.
Wearing a brace well
Line up the hinge with the side of the knee joint, keep straps snug but not cutting in, and check the skin daily. In the heat, a thin cotton layer underneath reduces sweating and rubbing. Do not change the hinge settings yourself; follow the plan from your surgeon or physiotherapist.
When to get help
Remove the brace and seek advice if your foot becomes numb, cold or discoloured, or if the calf swells or hurts, which can signal a blood clot. At Aman Hospital, Dr. Gelber explains at discharge whether you need a brace, the settings and when to stop.
Frequently asked questions
Can I sleep in my brace?
If your surgeon wants the knee kept straight at night in the early days, yes. Otherwise most people are allowed to remove it for sleep.
Should I buy an expensive custom brace for sport?
It is not needed for most players. Discuss it with your surgeon once you are near return to sport, rather than buying one early.
