Patient question

What is revision ACL surgery, and how is it different from the first operation?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

Revision ACL surgery is an operation to replace an ACL graft that has torn, stretched or failed to give stability. It is more complex than the first reconstruction because old tunnels, hardware and the reason for failure must be addressed, explains Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha, Qatar.

When it is considered

Revision is considered when the knee is unstable again after an earlier ACL reconstruction, for example after a new injury, a gradual loosening of the graft, or a graft that never fully took. Not everyone with a failed graft needs revision; the decision depends on symptoms, activity goals and the state of the rest of the knee.

What makes it different

  • The original bone tunnels may be in the wrong place, widened or still filled with screws
  • A different graft source is often needed because the first one has been used
  • The cause of failure must be found and corrected, such as tunnel position, bone shape or another loose ligament
  • The meniscus and cartilage are more often damaged by this stage
  • Sometimes the operation is done in two stages, with bone grafting of tunnels first

How it is planned

Planning usually includes a detailed examination, X-rays, often a CT scan to measure tunnels, an MRI, and the operation notes from the first surgery if available. Long-leg X-rays may be used to assess alignment, and the slope of the shin bone may be measured. Extra procedures, such as a lateral extra-articular tenodesis, are considered in some patients to improve rotational control.

What recovery looks like

Rehabilitation follows similar stages to a first reconstruction but is often more cautious, especially if bone grafting or meniscus repair is involved. At Aman Hospital in Doha, Dr. Gelber reviews every factor before recommending revision, so that the plan addresses why the first graft failed.

Frequently asked questions

Is revision ACL surgery common?

It is much less common than first-time reconstruction, but it is a recognised operation for knees that become unstable again after ACL surgery.

Can the same surgeon do the revision?

Yes, or a different surgeon can. What matters is careful assessment of why the first graft failed and experience with revision techniques.
