Patient question

Should I avoid squats forever if I have runner's knee?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

No. Squats are not harmful to the kneecap in themselves; the problem is usually too much depth, weight or volume too soon. Most people with runner's knee can squat again once the movement is reintroduced gradually, and doing so builds the strength that protects the joint, says Dr. Pablo Gelber, Doha knee surgeon at Aman Hospital.

Myth: squats wear out the kneecap

Load on the kneecap joint does rise as the knee bends deeper and the weight increases. That does not make squatting damaging. Cartilage and the surrounding tissues adapt to load that is built up steadily, and the quadriceps strength gained from squatting is one of the most useful protections for a painful kneecap. Avoiding it completely often leaves the thigh weaker and the knee more sensitive.

Myth: your knees must never pass your toes

Letting the knees travel forward shifts more load to the knee and less to the hip and back. During a painful phase, keeping the shin more upright and sitting back can be more comfortable. Once symptoms settle, there is no need to ban forward knee travel forever; daily life, stairs and prayer all involve it.

Myth: crackling when you squat means damage

Many people hear grinding under the kneecap when squatting. Without pain or swelling, this is very common and not a reason to stop.

How to bring squats back

  • Start with a box or chair squat to a depth that is comfortable
  • Use body weight before adding load
  • Increase depth first, then weight, one step at a time
  • Keep effort moderate and use the next-day response as your guide
  • Pair squats with hip and glute strengthening

If squats remain painful after several weeks of careful progression, or the knee swells or catches, see a knee specialist. At Aman Hospital in Doha, Dr. Gelber can check whether cartilage damage or another problem needs a different plan.

Frequently asked questions

Are leg extensions safer than squats?

Not necessarily. Both can be useful when the range and load are adjusted to what the knee tolerates.

Is it true I should never kneel again?

No. Kneeling is usually reintroduced gradually, sometimes with a cushion, as the pain settles.
