Gym, weights and CrossFit: training hard without hurting your knees
Quick answer
Squats, lunges and lifting strengthen the knee when the load rises gradually; most gym knee pain comes from doing too much too soon, poor depth control or high-volume jumping. Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha, sees tendon overload, kneecap pain and meniscus tears most, and assesses knees that swell, lock or give way.
Are squats bad for your knees?
For a healthy knee, no. Properly progressed squats, lunges and deadlifts build the muscles that protect the joint, and strength training is one of the main treatments for many knee conditions. Letting the knees travel forward past the toes is not dangerous in itself; it is a normal part of a deep squat for many people.
Problems arise when load, volume or depth increase faster than the tissues can adapt, or when an existing problem, such as a meniscus tear or cartilage damage, is loaded in positions that aggravate it. The question is rarely whether to squat, but how much, how deep and how often.
Common gym and CrossFit knee problems
- Patellar tendinopathy: pain just below the kneecap with squats, box jumps, double-unders and running in workouts.
- Patellofemoral pain: aching around the kneecap with deep squats, lunges, stairs and leg extension machines.
- Meniscus tear: a sharp pain on the inner or outer joint line, sometimes with a pop, in the bottom of a heavy or twisted squat; it may cause locking. Deep squatting under load is also linked to meniscal root tears in older lifters.
- Quadriceps or patellar tendon rupture: rare, but serious, usually when catching a heavy weight or landing awkwardly; you cannot straighten the leg.
- Box jump injuries: shin scrapes, falls and occasionally tendon or ligament injury, particularly when fatigued in timed workouts.
- Prepatellar bursitis: swelling over the front of the kneecap from kneeling on hard floors.
Warning signs to stop and get checked
- The knee locks or catches in a squat and you have to wiggle it to straighten
- Swelling after a session, especially if it is getting worse
- A pop with sudden pain, or the knee giving way
- Being unable to lift the straight leg after a heavy lift or landing, which needs same-day care
- Tendon pain that is worse each week despite lighter loads
After a very hard workout, severe muscle pain with dark urine needs urgent medical review, as it can signal muscle breakdown that affects the kidneys.
Programming and technique that protect the knee
- Increase load and volume gradually, and change one variable at a time
- Control depth: squat to a depth you can hold without pain or loss of position, then build from there
- Keep the knees tracking over the toes rather than collapsing inwards
- Limit high-rep jumping and running in the same week as heavy leg sessions, especially when starting out
- Step down from boxes rather than rebounding off them in high-volume sets
- Use a mat or pad for kneeling movements
- Plan lighter weeks, and return gradually after holidays, illness or Ramadan rather than resuming your previous loads
Training in Doha: gyms, heat and Ramadan
Gyms and CrossFit boxes in Doha are mostly indoor and air-conditioned, but some workouts include outdoor runs or carries, which in summer can bring on heat exhaustion and fatigue that makes technique break down. Move outdoor parts indoors or to cooler times.
During Ramadan, many people train after iftar or late at night. Training while dehydrated and short of sleep raises the risk of poor technique and injury, so reduce intensity, prioritise technique and keep heavy maximal lifts for later.
Returning to the gym after injury or surgery
The gym is often where rehabilitation happens, so the aim is not to stop training but to change it. Upper body and core work usually continue from early on. For the knee, early stages often use leg press, partial-range squats, step-ups and isometric holds that stay comfortable.
After knee arthroscopy for a partial meniscus removal, many people return to most gym training within several weeks. After meniscus repair, deep squatting under load is usually restricted for around three to four months to protect the repair. After ACL reconstruction, strength work starts early and progresses through a structured plan, while jumping and Olympic lifts are usually reintroduced later, once the knee tests well. Dr. Gelber and your physiotherapist will agree depth and load limits suited to the operation.
When to see a knee surgeon in Doha
See a specialist if your knee locks, swells, gives way, or keeps hurting despite reducing load for a few weeks, or immediately if you cannot straighten your leg after an injury. At Aman Hospital, Dr. Pablo Gelber examines the tendons, meniscus and kneecap, asks about your training in detail, and uses imaging when it will change the plan. Most gym-related knee pain is treated with programme changes and physiotherapy. A locked knee, a tendon rupture or a repairable meniscus tear may need surgery, sometimes urgently.
Frequently asked questions
Should I stop squatting if my knee hurts?
Often you only need to reduce load or depth. Pain that is mild and settles within 24 hours is usually acceptable during rehabilitation. Sharp pain, swelling or locking are reasons to stop and get assessed.
Is the leg extension machine bad for the knees?
Not for most people. It can aggravate kneecap pain at heavy loads, but at a comfortable range and weight it is a useful quadriceps exercise and is often used in rehabilitation.
Is CrossFit more dangerous for the knees than regular gym training?
Injury rates in CrossFit are broadly similar to other strength and fitness sports. Risk rises with fatigue, high-rep jumping and poor technique under time pressure, so scaling workouts matters.
Should I wear knee sleeves when lifting?
Sleeves keep the knee warm and many lifters find them comfortable. They do not prevent injury on their own, and they should not be used to train through a painful knee.
When can I squat again after meniscus repair?
Deep loaded squats are usually avoided for around three to four months after a repair, though shallower, lighter work starts earlier. Your surgeon will set the limits for your tear.
