Patient question

PCL tear: reconstruction or a brace and physiotherapy?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

Many isolated PCL tears, especially partial ones, do well with a dedicated PCL brace and quadriceps-focused rehabilitation. Reconstruction is more often advised when the PCL is torn with other ligaments, the shin bone sags markedly, or the knee stays unstable or painful despite rehab, explains Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha.

Why the PCL behaves differently from the ACL

The posterior cruciate ligament has more capacity to heal than the ACL, particularly when the shin bone is held in the right position while it heals. That is the purpose of a dynamic PCL brace, which pushes the top of the shin forwards.

The quadriceps muscle at the front of the thigh also helps hold the shin forward, so strong quadriceps can compensate for a looser PCL in daily life.

Brace and rehabilitation versus surgery side by side

  • Usually suits brace and rehab: isolated grade 1 or 2 tears, many isolated grade 3 tears, lower-demand activities
  • Usually suits reconstruction: PCL with posterolateral corner, MCL or ACL injury; a bony avulsion that can be fixed; persistent instability
  • Timeline: brace is often worn for several months; reconstruction recovery usually 9 to 12 months before full sport
  • Main drawback of non-surgical care: some knees stay loose, and long-term front-knee and inner-knee wear can develop
  • Main drawbacks of surgery: stiffness, residual looseness, rare nerve or vessel injury behind the knee
  • Flexibility: reconstruction can still be done later if rehabilitation does not settle symptoms

Combined injuries change the plan

PCL tears after car accidents or a fall onto a bent knee often come with other injuries, especially to the posterolateral corner. Those combinations rarely do well without surgery, and timing matters, so they are usually planned within the first few weeks.

When to see a knee surgeon in Doha

Book an assessment after a dashboard injury, a fall onto the front of a bent knee, or a tackle that left the back of the knee sore and swollen. Seek urgent care if your foot is cold, numb or weak. At Aman Hospital, Dr. Gelber measures how far the shin sags, checks the other ligaments and helps you decide between bracing and reconstruction.

Frequently asked questions

Can I play football again after a PCL tear without surgery?

Many people do, especially after isolated tears and good quadriceps rehabilitation. Return depends on strength and stability tests, not time alone.

Is a normal knee brace enough for a PCL tear?

Usually not. A dedicated PCL brace that supports the back of the shin is generally used for the first months.
