Patient question
My shin just below the knee hurts more every time I run. What is it?
Quick answer
Pain at the top of the shin that starts earlier in each run, sits in one tender spot and begins to ache at rest can be a stress reaction or stress fracture of the upper tibia. It needs rest from running and an assessment, says Doha knee surgeon Dr. Pablo Gelber at Aman Hospital in Qatar.
Why this pattern matters
Bone adapts to running by remodelling, but when training load rises faster than the bone can keep up, tiny areas of damage build up. At first this is a stress reaction; if loading continues, it can become a true crack in the bone.
The warning sign is the trend. Ordinary muscle soreness eases as you warm up and settles within a day or two. Bone stress pain tends to arrive sooner in each run, lingers afterwards, and eventually aches when walking or at night.
Stress fracture or shin splints?
- Shin splints usually cause a diffuse ache along a long stretch of the inner shin
- A stress fracture is usually tender at one small spot you can point to with a finger
- Pain on hopping on the affected leg points towards bone stress
- Swelling or warmth over the bone is more typical of a stress fracture
- Pain at rest or at night is a reason to stop running and be examined
What to do now
Stop running and jumping until the knee and shin have been assessed. Walking for daily needs is usually fine if it does not hurt; if it does, reduce it. You can keep fit with cycling or swimming if they are comfortable.
Avoid pain-killers that let you push through sessions. Think back over the last few weeks: a new race goal, more distance, harder surfaces or new shoes often explain the timing.
Getting it checked in Doha
Book an assessment if the pain has lasted more than a week or two, or sooner if you have pain at rest or cannot hop. At Aman Hospital, Dr. Gelber examines the knee and shin, and arranges X-rays and, when needed, an MRI, which shows early bone stress that X-rays often miss.
Seek urgent care if you cannot bear weight, or if pain follows a sudden crack during a run.
Frequently asked questions
Can I keep running slowly on it?
It is not advisable. Running on a bone stress injury can turn a reaction that heals with rest into a fracture that takes much longer.
Will a plain X-ray show it?
Often not in the first weeks. A normal X-ray does not rule out a stress injury, which is why an MRI is sometimes needed.
