Patient question
My knee swells up after I walk around the mall for a couple of hours. Why?
Quick answer
A knee that puffs up after a long walk is often an arthritic joint producing extra fluid when it is overloaded. It usually settles within a day or two with relative rest, ice and elevation. Repeated swelling deserves an examination to confirm the cause and adjust activity, advises Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha.
Why an arthritic knee swells after activity
The lining of the knee (the synovium) makes a small amount of lubricating fluid. When the cartilage is worn, longer or harder use irritates the lining, and it produces more fluid than the joint can clear. The knee feels tight, puffy and stiff, often worst in the evening or the next morning. Some people notice a soft bulge behind the knee, a Baker's cyst, which is linked to the same extra fluid.
Mall walking is a typical trigger in Qatar: hours on hard, flat floors, standing in queues and carrying bags add up to far more load than a normal day, even though it does not feel like exercise.
Settling a swollen knee
- Rest it relatively for a day or two, without stopping movement entirely
- Ice wrapped in a cloth for 15 to 20 minutes a few times a day
- Raise the leg above hip level when sitting
- Gentle bending and straightening keeps it from stiffening
- Simple pain relief or an anti-inflammatory gel can help, if suitable for you
Preventing the next flare
You do not need to avoid malls, which are one of the most practical places to walk during the hot months. Pace the visit: sit for a few minutes every half hour, wear cushioned, supportive shoes and spread errands over more than one trip. Build tolerance gradually, adding a few minutes to your walks each week, and work on thigh strength so the muscles absorb more of the load. If you are carrying extra weight, losing even a modest amount reduces the force through the knee with every step.
When swelling needs a doctor
Book an assessment if swelling happens after most walks, lasts more than a few days, or comes with locking or giving way. A doctor may drain fluid and test it, and a steroid injection can calm a persistent flare.
Seek urgent care the same day if the knee is hot, red and very painful, if you have a fever, or if the calf is swollen and tender, which can signal infection, gout or a blood clot. At Aman Hospital, Dr. Gelber examines the knee, reviews X-rays and, if needed, an ultrasound or MRI, and plans treatment around how active you want to be.
Frequently asked questions
Should I wear a knee sleeve when I walk?
A simple elastic sleeve gives some people comfort and a feeling of support. It does not treat the arthritis, but it is reasonable to use if it helps you walk more.
Is it harmful to keep walking if my knee swells?
Walking is good for arthritic knees, but repeated swelling means the dose is too high for now. Shorten the walks, build up slowly and strengthen the muscles.
