Patient question

My knee swells the day after running. Should I stop?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

Swelling that shows up the day after a run means the joint itself is irritated, often by a meniscus tear, a cartilage problem or early arthritis, rather than tired muscles. Ease back rather than push through, and get it checked if it keeps happening, advises Dr. Pablo Gelber, knee surgeon at Aman Hospital, Doha.

Why delayed swelling is a useful signal

Muscle soreness after a run is felt in the thigh or calf and does not make the knee look bigger. True swelling, a puffy area above or beside the kneecap or a tight feeling behind the knee, means the joint lining has produced extra fluid. When that happens hours later rather than straight away, it usually points to something inside the joint being irritated by repeated impact rather than a sudden tear.

Common culprits are a small degenerative or flap tear of the meniscus, a patch of worn or damaged joint cartilage, and early osteoarthritis. Pain on the front of the knee without much swelling is more often runner's knee (patellofemoral pain), and pain on the outer side is often the IT band.

Training changes that usually help

  • Cut your weekly distance and long-run length for two to three weeks, rather than stopping all exercise
  • Keep fitness with cycling, swimming or the cross-trainer
  • Avoid downhill running and hard surfaces while the knee settles
  • In Qatar, run in the cooler early morning or on an indoor track; heat and dehydration make it easier to overdo a session
  • Build back gradually, increasing distance slowly from week to week
  • Add hip and thigh strengthening twice a week; it reduces load on the knee

What to do the morning after

Ice the knee for 15 to 20 minutes, keep it moving gently and rest it raised for a while. If the swelling is gone within a day and the knee bends and straightens fully, an easier week is usually enough. If it is still swollen after two to three days, or you notice catching, clicking with pain, or the knee giving way, stop running until it has been examined.

When to see a knee surgeon in Doha

Book a review if swelling follows most runs for more than a few weeks, or if you have pain on the inner or outer joint line, locking or instability. Dr. Gelber examines how the knee moves and where it is tender, assesses alignment and running load, and arranges an MRI when a meniscus or cartilage injury is suspected. Most runners are managed with a structured rehabilitation plan; a few with a mechanical problem benefit from keyhole surgery.

Frequently asked questions

Will running give me arthritis?

In people with healthy knees, recreational running has not been shown to cause arthritis and may even be good for cartilage. Running on a knee that keeps swelling is different, because it suggests the joint is already irritated.

Should I wear a knee brace to run?

A brace does not fix the cause of the swelling. A sleeve may feel comfortable, but the priority is reducing load and finding out why the knee is reacting.
