Patient question
My knee bent backwards and now it feels completely unstable. Have I torn more than one ligament?
Quick answer
A knee forced backwards that now feels loose in more than one direction may have several torn ligaments, often the ACL, PCL and a collateral ligament, and may even have dislocated briefly. It needs urgent examination, including checks of the artery and nerves, advises Dr. Pablo Gelber, multi-ligament knee surgeon at Aman Hospital in Doha.
Why it feels so unstable
Each ligament controls a particular direction of movement. The ACL limits the shin sliding forward and rotating, the PCL limits it sliding backward, and the collateral ligaments limit opening on the inner and outer sides. Severe hyperextension can tear the ACL and PCL together, and often the back of the capsule and the posterolateral corner too. When two or more of these fail, the knee can feel as though it moves in every direction, which patients often describe as jelly-like or as if the leg might bend the wrong way.
Signs that this needs emergency care first
- The knee looked out of place, even briefly, or someone saw it deformed
- The foot is cold, pale, numb or weak
- Calf pain or swelling that is getting worse
- The injury happened in a road traffic accident, a fall from height or a high-speed sports collision
If any of these apply, go to an emergency department now or call 999 in Qatar. A multi-ligament injury of this kind is treated as a possible knee dislocation until proven otherwise.
If circulation and nerves are normal
Keep the knee in a brace or splint as advised, use crutches and avoid letting the knee sag backward when you stand. Swelling and bruising may spread down the calf over a few days, which is common. An MRI is usually arranged to show exactly which ligaments, tendons and menisci are torn.
Getting a specialist plan in Doha
Multi-ligament knee injuries are uncommon and complex, and the order and timing of surgery matter. At Aman Hospital, Dr. Gelber examines each ligament in turn, reviews the MRI and stress X-rays and explains whether early repair, staged reconstruction or, occasionally, bracing alone is most suitable for your injury and goals.
Frequently asked questions
Can several torn ligaments heal without surgery?
Some components, such as a partial MCL tear, can heal in a brace, but most knees with two or more complete ligament tears remain unstable without reconstruction. The plan depends on which ligaments are involved.
Will my knee ever feel normal again?
Many patients regain a stable, functional knee for daily life and work after reconstruction and long rehabilitation. Return to high-level pivoting sport is possible for some, but not all, and is discussed honestly before surgery.
