Patient question
What exercises help a meniscus tear without surgery, and which should I avoid?
Quick answer
A good non-surgical programme for a meniscus tear calms the knee first, then builds thigh and hip strength, balance and controlled bending over roughly 6 to 12 weeks, while avoiding deep loaded squats and pivoting. It suits most stable and degenerative tears, explains Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha.
Stage 1: settle the knee
For the first one to two weeks, the aim is less swelling and full straightening. Typical exercises include:
- Quadriceps sets: tighten the thigh with the leg straight and hold for five seconds
- Straight-leg raises lying down
- Heel slides, bending only as far as is comfortable
- Gentle stationary cycling with the seat set high
Stage 2: build strength and control
Once walking is comfortable, the work becomes more demanding:
- Mini squats and sit-to-stand from a chair, keeping the knee over the second toe
- Step-ups onto a low step
- Bridges and side-lying leg raises for the hip muscles
- Single-leg balance, progressing to a cushion or wobble board
- Leg press within a comfortable range rather than full depth
A little discomfort during exercise is acceptable if it settles within a day and swelling does not increase.
What to avoid during the programme
- Deep squats under load, especially with a barbell
- Twisting on a planted foot, such as football, padel or basketball
- Kneeling and sitting on your heels until bending is comfortable
- High-impact running or jumping until single-leg strength is close to the other side
How long to give it, and when to reconsider
Many patients with degenerative tears notice a real difference by six to twelve weeks. If the knee still catches, locks, swells repeatedly or gives way despite a well-done programme, or if a younger patient has a tear likely to be repairable, Dr. Gelber discusses whether arthroscopy makes sense. A supervised physiotherapist in Doha makes the programme safer and more effective than exercising alone.
Frequently asked questions
Can supplements or injections replace exercise?
No supplement has been shown to heal a meniscus. An injection can occasionally calm a painful flare so that exercise becomes possible, but it is an aid to rehabilitation, not a substitute for it.
Is swimming good for a meniscus tear?
Swimming and water walking are usually well tolerated. Avoid breaststroke kick, which twists the knee, until the knee is settled.
