Patient question
Is it too late to fix my stiff knee months after the operation?
Quick answer
Usually not. Early treatment is simpler, but many knees that are still stiff months or even years after surgery can be improved, often with arthroscopic release of mature scar tissue followed by focused physiotherapy. The first step is finding why the knee is stiff, advises Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha, Qatar.
What changes over time
In the first few months, scar tissue is softer and more responsive to physiotherapy and manipulation. After about three to six months it matures, becoming denser and tougher, so stretching alone gains less and manipulation alone carries more risk.
That does not mean nothing can be done. It means the approach usually shifts from forcing movement to surgically releasing the scar and then protecting the gain.
Questions a fresh assessment answers
- Is the problem mainly loss of straightening, bending, or both?
- Is there a specific block, such as a cyclops nodule, a graft in a poor position or a loose body?
- Is the kneecap tethered or sitting too low?
- Is there ongoing inflammation or low-grade infection?
- How much movement do you need for your work, prayer, sport and daily life?
Treatment options at this stage
Arthroscopic arthrolysis, releasing and removing scar tissue through small incisions, is the most common approach for established stiffness. A specific mechanical cause, such as a malpositioned graft or implant, may need revision surgery. In a few severe cases, an open release is needed.
Whatever the procedure, intensive physiotherapy afterwards is essential, and you should plan time for it.
Getting seen in Doha
If your knee is still stiff and limiting you, it is reasonable to ask for a review or a second opinion. At Aman Hospital, Dr. Gelber examines the knee, reviews your operation notes and scans, and arranges new imaging if needed. Bring any reports and the name of the operation you had; this saves time and helps plan treatment.
Frequently asked questions
Will I get all my movement back?
It depends on the cause and how long it has been stiff. Many patients gain meaningful movement, but the goal is set honestly after examination.
My surgery was abroad. Can I still be assessed in Doha?
Yes. Bring your operation notes, implant details and scans if you have them; they help, but they are not essential to start.
