Patient question

My knee swelled straight away, or only the next day: what is the difference?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

Swelling within the first few hours usually means bleeding inside the knee, often from an ACL tear, a kneecap dislocation or a fracture. Swelling that builds overnight is more often joint fluid from irritation, typical of a meniscus tear or sprain. Rapid swelling needs prompt assessment, explains Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha.

Blood or fluid

When a structure with a good blood supply tears inside the joint, blood fills the knee quickly. The knee becomes tense, warm and hard to bend within an hour or two.

When the injury is to a structure with less blood supply, or the lining of the joint is irritated, the knee makes extra joint fluid. That happens more slowly and is often noticed the next morning.

Immediate and delayed swelling side by side

  • Timing: immediate within about 2 hours; delayed over 12 to 24 hours
  • What it usually is: immediate blood in the joint; delayed joint fluid
  • Common causes of immediate swelling: ACL tear, kneecap dislocation, fracture, peripheral meniscus tear
  • Common causes of delayed swelling: meniscus tear, cartilage injury, ligament sprain, flare of arthritis
  • How it feels: immediate tense, tight and very painful to bend; delayed puffy and stiff
  • Urgency: immediate swelling should be seen within days; delayed swelling if it persists or comes with locking or giving way

What to do now

Rest the knee, use ice in short spells, keep it raised and use crutches if needed. Avoid football, padel and the gym until it is assessed. Swelling with no injury, or a hot red knee with fever, is different and may mean infection or gout, which needs urgent care.

When to see a knee surgeon in Doha

Book an assessment within a few days if your knee swelled within hours of an injury, even if it now feels better. Seek urgent care for a deformed knee, inability to bear any weight, a numb or cold foot, or fever. At Aman Hospital, Dr. Gelber examines the ligaments, meniscus and kneecap and arranges X-rays or MRI when needed.

Frequently asked questions

Should my swollen knee be drained?

Sometimes, if it is very tense and painful or infection must be ruled out. Many swollen knees settle without drainage.

Does a lot of swelling mean a worse injury?

Not always, but fast, tense swelling raises the chance of a significant injury such as an ACL tear.
