Patient question

Is it my ACL or my meniscus? How do the symptoms differ?

Quick answer

An ACL tear usually brings a pop, rapid swelling and later a knee that gives way when you turn. A meniscus tear more often causes pain at one point along the joint line, catching or locking, and slower swelling. The two often occur together, so examination matters, says Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha.

Instability versus mechanical symptoms

The ACL is a stabiliser, so its main symptom is the knee shifting or giving way, especially when you change direction. The meniscus is a shock absorber between the bones, so a tear usually causes localised pain and mechanical symptoms: catching, clicking with pain, or the knee getting stuck.

After a sports injury both can swell and hurt, which is why the first days are confusing. The pattern becomes clearer as the initial swelling settles.

ACL and meniscus symptoms side by side

  • The moment of injury: ACL often a loud pop and you fall; meniscus often a twist while bent, sometimes with no clear pop
  • Swelling: ACL usually within a few hours from bleeding; meniscus often over 12 to 24 hours
  • Where it hurts: ACL deep inside and hard to point to; meniscus along the inner or outer joint line
  • Typical complaint: ACL the knee feels loose or slips; meniscus the knee catches, locks or hurts when squatting
  • Without injury: ACL tears almost always need a clear injury; meniscus tears can appear with wear in people over about 40

Why both are often torn together

The forces that tear the ACL often trap the meniscus between the bones. An ACL-deficient knee that keeps slipping can also tear the meniscus later. This matters because a meniscus tear found early is more often repairable, which can be done during ACL reconstruction.

When to see a knee surgeon in Doha

Book an assessment if your knee swelled after a twist in football, padel or the gym, if it gives way, or if it catches or locks. A knee that is locked and cannot fully straighten should be seen within days, since a displaced meniscus tear may need prompt repair. At Aman Hospital, Dr. Gelber tests ligament stability and the joint line and arranges an MRI when needed.

Frequently asked questions

Can I have an ACL tear without much pain?

Yes. Once the early swelling settles many people walk with little pain. Giving way when turning is often the main clue.

Does a meniscus tear make the knee give way?

It can cause a brief buckling from pain or catching, but true slipping of the shin bone is more typical of a ligament injury.
