Patient question

How tight should my knee brace be, and why does it keep slipping?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

A knee brace should feel snug enough not to slide, but you should still fit a finger or two under each strap, and the leg below must not swell, tingle or change colour. A brace that keeps slipping is usually the wrong size or badly positioned, says Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha.

Getting the fit right

  • Measure as the brace maker instructs, often the leg circumference above and below the kneecap
  • For a hinged brace, line the hinges up with the side of the knee joint, not above or below it
  • Fasten the straps closest to the knee first, then the outer straps
  • Check you can slide one or two fingers under each strap
  • Sit down and stand up a few times, then readjust

Why braces slip

Slipping is common when the size is too large, the hinges are positioned too low, the thigh is cone-shaped or muscle has thinned after injury. Sweat in hot weather also makes braces slide. A thin non-slip layer underneath, tightening the thigh straps a little more than the calf straps and refitting after swelling settles all help.

After surgery, a brace that sits too low can let the knee bend more than intended, so ask for a refit if it keeps moving.

Signs it is too tight

Loosen it at once and seek advice if you notice tingling, numbness, pins and needles, a cold, pale or blue foot, or swelling of the ankle and foot below the brace. Deep marks or blisters under straps also mean it needs adjusting. Calf pain or swelling, especially after surgery or a long flight, needs urgent review for a blood clot.

Help with fitting

Your physiotherapist or the clinic team can check the size and position, especially after surgery when swelling changes from week to week. At Aman Hospital, Dr. Gelber and the team review brace fit at follow-up and explain when to change settings or stop wearing it.

Frequently asked questions

Should I tighten the brace more for sport?

Slightly firmer may feel more secure during activity, but the same safety rules apply. Loosen it again afterwards.

Do I need a new size as swelling goes down?

Sometimes. Many braces adjust with straps, but if it keeps slipping once swelling settles, a smaller size may fit better.
