Patient question
Does a PCL tear heal without surgery?
Quick answer
Yes, in many patients. Isolated grade I and II PCL tears, and some grade III tears, often heal well with a dedicated PCL brace and quadriceps-focused physiotherapy, because the PCL has a better healing capacity than the ACL. Combined ligament injuries usually need surgery, explains Dr. Pablo Gelber at Aman Hospital in Doha.
Why the PCL can heal
The PCL has a good blood supply and is surrounded by soft tissue, so the torn fibres can scar back together. The challenge is that it may heal in a stretched position if the shin is allowed to sag backwards during healing. That is why non-surgical treatment focuses on holding the tibia forward while the ligament repairs.
What non-surgical treatment involves
- A dynamic PCL brace that supports the back of the upper shin, often worn for several months for higher-grade tears
- Early quadriceps strengthening, because this muscle pulls the tibia forward and protects the PCL
- Avoiding isolated hamstring exercises early on, as hamstrings pull the shin backwards
- Gradual return of full bending, usually protected in the first weeks
- Balance, gait and later running and agility work
Rehabilitation is usually supervised by a physiotherapist with regular review by Dr. Gelber to check that the knee is not sagging.
When surgery is usually recommended
Surgery is considered when the PCL tear is combined with injury to the posterolateral corner, the medial ligament or the ACL, when a bone fragment has pulled off with the ligament, or when the knee remains unstable and symptomatic despite good rehabilitation. People with a chronic PCL tear who develop progressive pain may also be assessed for reconstruction.
Sticking with the programme in Qatar
Consistency matters more than intensity. In the summer heat, plan walking and conditioning indoors or in the cooler hours, and keep up home exercises during busy periods such as Ramadan, when gentle sessions after iftar may be easier to manage. If the brace is uncomfortable in the heat, a breathable sleeve underneath often helps.
Frequently asked questions
How long will I need the PCL brace?
It depends on the grade. Lower-grade tears may need a brace for a shorter period, while complete tears are often braced for around three to four months. Dr. Gelber sets the duration after examination.
Can I return to sport without surgery?
Many athletes with isolated PCL tears return to sport after rehabilitation, often within a few months for lower grades. Quadriceps strength is the key to a safe return.
