Patient question

Why does my knee swell every evening and look normal in the morning?

Quick answer

A knee that fills up by evening and settles overnight is usually producing extra joint fluid in response to the day's load. Early arthritis, a degenerative meniscus tear or a cartilage problem are the usual causes. If both ankles and legs swell too, think of circulation first, says Doha knee surgeon Dr. Pablo Gelber at Aman Hospital.

What is going on inside the knee

The lining of the knee, the synovium, makes a small amount of fluid to lubricate the joint. When something inside is irritated, such as worn cartilage or a frayed meniscus, the lining responds to each step by making more. Through the day that fluid builds up, so by evening the knee feels tight, full or puffy, especially at the back or above the kneecap. Lying down overnight lets it be reabsorbed, so the morning knee looks better but may feel stiff for the first few minutes.

This pattern points to a mechanical or wear-related cause rather than an infection, and it is a sign that the knee is being loaded more than it can comfortably handle at the moment.

Is it the knee or the whole leg?

  • Swelling only in the knee, often with a fullness around the kneecap: usually a joint problem
  • Swelling of the ankles and lower legs on both sides that leaves a dent when pressed: more likely fluid retention from veins, heart, kidneys or some medicines, which your GP should check
  • Swelling of one whole calf and leg with tenderness: could be a clot and needs urgent assessment

What helps day to day

  • Break up long periods of standing or walking, and sit with the leg raised for a few minutes during the day
  • Ice for 15 to 20 minutes in the evening
  • Strengthen the thigh and hip muscles, which take load off the joint; a physiotherapist can set a simple plan
  • Choose cushioned, supportive shoes, particularly on hard floors or long shifts
  • If you are carrying extra weight, even modest weight loss lowers the load on each step
  • Swap high-impact exercise for cycling or swimming for a while

When to see a knee surgeon in Doha

If the evening swelling continues for more than a few weeks, is getting worse, or comes with catching, locking or pain on the inner or outer side of the knee, an assessment is worthwhile. Dr. Gelber examines the knee and usually starts with a standing X-ray, adding an MRI when a meniscus tear or cartilage injury is suspected. Most patients with this pattern do well with physiotherapy, activity changes and sometimes an injection. Surgery is considered when there is a clear mechanical cause that is not settling.

Frequently asked questions

Why is my knee stiff in the morning if the swelling has gone?

Joints that have been still for hours stiffen, especially with arthritis. Morning stiffness from wear usually eases within about half an hour of moving. Stiffness lasting much longer may suggest an inflammatory arthritis.

Should I wear a knee sleeve?

A simple compression sleeve can make the knee feel more supported and may limit some swelling during the day. It is a comfort measure, not a treatment, so keep up the exercises.
