Patient question

Does running in Qatar make IT band syndrome worse?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

Running in Qatar does not cause IT band syndrome, but local patterns can trigger it: months on a treadmill followed by a sudden jump in outdoor mileage when the weather cools, cambered roads and repeated loops in one direction. Planning those transitions helps, advises Doha knee surgeon Dr. Pablo Gelber.

The seasonal spike

Many runners in Doha train indoors or very early in the morning through the hot months, then increase distance quickly when outdoor running becomes pleasant again and race season approaches. That jump in weekly volume is one of the most common triggers of IT band pain we see. Build mileage gradually in the autumn, rather than trying to catch up in a few weeks.

Treadmill to road

A treadmill is flat, predictable and slightly cushioned. Roads and running paths add camber, kerbs and small gradients, and your stride changes. When moving outdoors, start with shorter runs than you were doing on the treadmill and let the knee adapt over two to three weeks.

Routes and loops

Popular routes such as waterfront promenades, park loops and running tracks are often run in the same direction every time. On a cambered path, one leg is always on the lower side, and on a track the inside leg works differently on the bends. Switch direction regularly, vary routes and include some flat surfaces when you are recovering.

Heat, hydration and Ramadan

Fatigue changes running form, and heat brings fatigue on sooner. Tired hip muscles let the knee drift inwards, which loads the IT band. Run in the cooler hours, hydrate well and shorten sessions on hot or humid days.

During Ramadan, many runners train after iftar or before suhoor with less sleep. It is a sensible time to maintain fitness with shorter, easier runs rather than increase distance.

When to be seen

If outer knee pain persists despite changing training, or you have swelling, locking or pain after a twist, book an assessment at Aman Hospital so other causes can be excluded.

Frequently asked questions

Is the treadmill safer for my IT band?

It is a useful way to control pace and surface while you recover, but you will still need to adapt to outdoor running gradually.

Does running on sand help?

Soft, uneven sand is harder work for the hip and knee and is better avoided until symptoms settle.
