Patient question

Why do I have pain just below the inside of my knee when I climb stairs?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

A tender spot two to five centimetres below the inner knee joint, sore climbing stairs or getting out of a car, is typical of pes anserine bursitis or tendinopathy, where three tendons attach to the shin. It is common with knee arthritis, diabetes and extra weight, explains Dr. Pablo Gelber, knee surgeon at Aman Hospital, Doha.

What the pes anserine is

Three tendons from the inner thigh and the back of the thigh join and attach together on the upper inner shin. The name means goose's foot, after their shape. A small fluid sac, the bursa, lets them glide over the bone.

Climbing stairs, rising from a chair and getting in and out of the car all work these tendons with the knee bent. When the area is irritated, the pain is precise: you can usually put one finger on it, below the joint line rather than on it.

Who tends to get it and what else it could be

  • Adults with inner-knee osteoarthritis, where altered walking overloads the tendons
  • People with type 2 diabetes or extra weight, both common in Qatar
  • Runners and cyclists who increase hill work or distance quickly
  • Medial meniscus tear or arthritis can coexist, with pain on the joint line itself
  • A stress fracture of the upper tibia is less common but causes pain that builds with each step and lingers at rest

What helps

Use the rail and lead up the stairs with the good leg for a few weeks. Ice for 15 minutes after activity, and sleep with a pillow between the knees if lying on your side presses on the spot. Anti-inflammatory gel on the tender area helps some people when it is safe for them.

Physiotherapy targets the hamstrings, inner thigh and hip muscles with gradual strengthening and stretching, and addresses the knee arthritis if it is the driver. If you have diabetes, good glucose control is part of recovery. Symptoms commonly improve over six to twelve weeks. A steroid injection into the bursa is sometimes used when pain persists, but it does not treat an underlying joint problem.

When to see a knee surgeon in Doha

Book an assessment if the pain persists beyond six weeks, if there is swelling of the joint, or if the knee catches or gives way, which suggest a problem inside the joint as well. See a doctor quickly for a hot, red swelling, fever, or pain that steadily worsens even at rest.

At Aman Hospital, Dr. Gelber distinguishes pes anserine pain from meniscus and joint problems by careful examination. Ultrasound can show the bursa, and X-rays look for arthritis that may be driving it.

Frequently asked questions

Is pes anserine bursitis serious?

No, it is a common overload problem and usually settles without surgery. The main point is to check for an associated joint problem such as arthritis or a meniscus tear.

Why does it hurt when my knees touch at night?

Lying on your side can press one knee's tender spot against the other. A pillow between the knees removes that pressure.
