Patient question

Why does the outside of my knee hurt when I cycle?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

When the outer knee aches during or after a ride, the cause is usually IT band irritation or a strained tendon on the outside of the knee, very often driven by bike setup: a saddle that is too high, cleats rotated the wrong way or a sudden rise in distance, says Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha.

Why cyclists get outer knee pain

Cycling repeats the same bend and straighten thousands of times per ride. If something in the setup pulls the knee slightly out of line, the soft tissues on the outer side take the strain each pedal stroke. The IT band, which crosses the outer knee, and the biceps femoris tendon, the outer hamstring that attaches near the top of the fibula, are the usual sources.

Typical culprits are a saddle set too high or too far back, so the leg overreaches at the bottom of the stroke; cleats that turn the foot inward and force the knee outward; very narrow pedal stance; and a fast increase in kilometres, for example joining group rides or spin classes several times a week.

Adjustments to try first

  • Lower the saddle a few millimetres at a time and check that the knee keeps a slight bend at the bottom of the stroke
  • Recheck cleat rotation so your foot sits in its natural angle, and consider pedals with more float
  • Reduce distance and intensity for one to two weeks, and keep cadence light rather than grinding big gears
  • Add hip and glute strengthening off the bike
  • If symptoms persist, a professional bike fit is often worthwhile

Signs it is not just a fit problem

Pain that started after a fall from the bike, swelling of the knee, a feeling that the knee catches or locks, or pain that wakes you at night are not typical of simple overuse. Numbness or tingling down the outer shin, or difficulty lifting the front of the foot, can point to irritation of the peroneal nerve near the fibula and should be checked promptly.

When to see a knee specialist

If outer knee pain continues after you have corrected the setup and reduced load for four to six weeks, or if any of the signs above appear, book an assessment with a knee surgeon in Doha. Dr. Gelber examines the IT band, tendons, ligaments and lateral meniscus, and arranges imaging only if the examination suggests a structural problem.

Frequently asked questions

Is cycling bad for my knees?

No. Cycling is usually one of the kinder exercises for the knee. Pain on the bike is far more often about setup and load than damage.

Should I stop cycling completely?

Usually not. Shorter, easier rides with a corrected setup often let the tissues settle while you stay active.
