Patient question

My knee is stiff for over an hour every morning. Is that arthritis?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

Wear-and-tear arthritis usually stiffens the knee for less than half an hour after waking. Stiffness that lasts an hour or more, especially with swelling or other painful joints, raises the possibility of an inflammatory arthritis that needs blood tests and specialist care, explains Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha.

Why the length of stiffness matters

Doctors pay close attention to how long morning stiffness lasts, because it helps separate two broad groups. In osteoarthritis, the knee is stiff for a few minutes, sometimes up to around half an hour, and then loosens with walking. In inflammatory conditions, the joint lining itself is inflamed, fluid builds up overnight and stiffness often lasts an hour or longer.

Neither is a firm rule, but stiffness that takes most of the morning to ease deserves a closer look.

Possible causes

  • Rheumatoid arthritis, often with stiff, swollen hands, wrists or feet on both sides
  • Psoriatic arthritis, sometimes with skin psoriasis or nail changes
  • Spondyloarthritis, with back stiffness that improves with movement
  • Gout or pseudogout, typically with flares of a hot, swollen joint
  • Reactive arthritis after a gut or urinary infection
  • More advanced osteoarthritis with a large amount of fluid in the knee

What helps in the meantime

A warm shower and gentle bending and straightening before you get up often shorten the stiffness. Keep active through the day with walking or cycling, and avoid long spells sitting still. Note which joints are stiff, how long it lasts and whether the knee is swollen; a simple diary helps your doctor.

If you fast during Ramadan, discuss the timing of any regular medicines with your doctor in advance rather than stopping them.

Who to see in Doha

Seek prompt care if a single knee suddenly becomes hot, red and very swollen, or you have a fever. Book an assessment if morning stiffness lasts more than half an hour on most days for several weeks, or if other joints are involved.

At Aman Hospital, Dr. Gelber examines the knee, looks for fluid and arranges X-rays and blood tests where needed. If an inflammatory arthritis is likely, he will advise review with a rheumatologist, because early medical treatment protects the joints; the knee surgeon's role then is to manage mechanical problems.

Frequently asked questions

Can young people have long morning stiffness?

Yes. Inflammatory arthritis can start in the twenties and thirties, so age alone does not rule it out. Persistent stiffness with swelling should be checked whatever your age.

Will an X-ray show inflammatory arthritis?

Early on it may look normal. Blood tests, examination and sometimes ultrasound or MRI help make the diagnosis before damage is visible on X-ray.

Should I take anti-inflammatory tablets every morning?

Short courses help some people, but regular use should be supervised by a doctor, particularly if you have stomach, kidney or heart problems.
