Patient question

My knee is stiff after sitting in the car. Why, and what helps?

Quick answer

Stiffness after a drive usually comes from holding the knee bent for a long time, which presses the kneecap into its groove and lets joint fluid settle. It is typical of kneecap pain and early arthritis, and simple changes help, says Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha, Qatar.

Why the car seat is hard on knees

Many people in Qatar spend long periods in the car, whether in Doha traffic or driving to Al Khor, Dukhan or Mesaieed for work. The knee is held bent, often with the right foot working the pedals and little chance to move. That position keeps steady pressure behind the kneecap and lets the joint gel, so the first steps out of the car feel stiff and achy.

The same thing happens in cinemas, long meetings and at a desk, and doctors sometimes call it the theatre sign.

Likely causes

  • Patellofemoral pain or softened cartilage behind the kneecap, common in younger adults
  • Early osteoarthritis, with stiffness that eases after a few steps
  • Tight quadriceps and hamstrings from long hours of sitting
  • Mild swelling from a recent injury or meniscus problem
  • After knee surgery, the joint naturally stiffens more when held still

What helps on the road and at work

  • Move the seat back so the knee is less sharply bent and your foot reaches the pedals without stretching
  • On long drives, stop roughly every hour or two to walk for a few minutes
  • While stopped or as a passenger, straighten and bend the knee and tighten the thigh muscles
  • Use cruise control on suitable roads to rest the right leg
  • At a desk, stand or walk briefly every 30 to 45 minutes
  • Strengthen the hip and thigh muscles, which reduces pressure behind the kneecap

When to get it checked in Doha

Book an assessment if the stiffness takes more than a few minutes to ease, if the knee swells, catches or locks, or if pain is changing how you walk. Seek urgent care for calf pain and swelling after a long journey.

At Aman Hospital, Dr. Gelber examines the kneecap, the joint lines and the muscle balance, and arranges an X-ray if arthritis is suspected. Most patients improve with a targeted exercise programme.

Frequently asked questions

Is it safe to drive with a stiff knee?

If you can move the pedals quickly and with full control, usually yes. After surgery on the right knee, check with your surgeon and insurer before driving again.

Why is the stiffness worse in the evening after work?

Hours of sitting and a long drive home add up. Breaking up sitting during the day often makes the evening stiffness much milder.

Does an automatic car help?

For a painful left knee, an automatic reduces clutch use. For the right knee, both types use the pedals, so seat position and breaks matter more.
