Patient question

My knee feels weak and wobbly since my injury. Is something torn?

Quick answer

Weeks after an injury, a weak, wobbly knee is often the thigh muscle switching off in response to pain and swelling, which recovers with targeted exercise. If it also shifts when you turn or swelled quickly at the time, a torn ligament is more likely, says Dr. Pablo Gelber, knee surgeon at Aman Hospital, Doha.

Weak is not always the same as loose

After any knee injury, swelling inside the joint sends signals that partly shut down the quadriceps. Doctors call this arthrogenic muscle inhibition. The muscle can shrink visibly within a couple of weeks, and the knee then feels shaky, tired and unreliable, even if no ligament is torn.

A loose knee is different. The joint itself moves too much: it shifts or slides when you turn, land or step sideways. Both feelings can exist together, which is why an examination matters.

Clues that point to a structural injury

  • You heard or felt a pop at the moment of injury
  • The knee swelled up within a few hours
  • You could not continue playing
  • The knee shifts or gives way when you turn, not only when tired
  • It catches, locks or will not fully straighten
  • The wobble is side to side after a blow to the inner or outer knee

What you can do now

Keep the knee moving and work on the muscle every day. Tightening the thigh with the leg straight, straight-leg raises, gentle mini-squats and a stationary bike are usually safe. If you can, compare your two thighs in a mirror; a smaller thigh on the injured side is a sign that strength work is needed.

Avoid football, padel and other turning sports until the cause is clear. Returning to sport on a knee that feels wobbly increases the risk of a second, more serious injury.

When to be seen in Doha

Book an assessment if the knee still feels unstable four to six weeks after the injury despite exercise, or sooner if any of the clues above apply. Seek urgent care for a knee that cannot take weight, is visibly deformed, or has numbness or a cold foot.

At Aman Hospital, Dr. Gelber examines each ligament, the meniscus and muscle strength. An MRI is requested when the examination suggests a tear, and physiotherapy is the first step when it does not.

Frequently asked questions

How long does a knee take to feel strong again after a sprain?

For a mild sprain, many people feel steady again within a few weeks of active rehabilitation. Muscle that has wasted takes longer to rebuild, often a couple of months of regular work.

Can a torn ACL be missed at the first visit?

It can, because swelling and pain make early examination difficult. If the knee still feels unstable once the swelling settles, a repeat examination is worthwhile.

Is a knee sleeve helpful for a wobbly knee?

A simple sleeve may improve your sense of the knee's position and your confidence. It does not replace strengthening, and it will not hold a torn ligament in place.
