Patient question

My knee feels tight and stiff after football or the gym. Is that normal?

Quick answer

Muscle tightness around the knee for a day or two after hard training is normal. Stiffness with a puffy knee, pain on the joint line, or catching is not, and suggests fluid inside the joint from a meniscus, cartilage or kneecap problem, explains Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha, Qatar.

Muscle soreness or a joint problem?

After a new or heavier session, the quadriceps and hamstrings can feel tight and sore for 24 to 72 hours. This delayed muscle soreness is felt in the muscle bellies above and behind the knee, eases with gentle movement and leaves the knee itself looking normal.

Joint stiffness feels different: the knee feels full or tight inside, bending is limited, and you may see swelling around the kneecap compared with the other side. That pattern means the joint lining has reacted to something.

Common causes of joint stiffness after sport

  • A meniscus tear, especially after twisting in football, with pain on the inner or outer joint line
  • Cartilage wear or a cartilage defect, causing an ache and swelling the day after impact
  • Kneecap overload from deep squats, lunges or jumping
  • Patellar tendon irritation, with stiffness below the kneecap first thing in the morning
  • Training too much, too soon, or in the heat with poor recovery

What helps

For simple muscle tightness, light cycling, walking and stretching the next day help more than complete rest. Build training load gradually, roughly 10 percent a week, and include rest days.

If the knee is puffy, cut back impact and deep bending for a week, use ice and keep up gentle range-of-motion work. In Qatar's hot months, train in the early morning, late evening or indoors, and hydrate well; heat and fatigue make poor technique more likely.

When to have it checked in Doha

Book an assessment if the knee swells after most sessions, catches or locks, gives way, or the stiffness lasts more than a few days. Seek prompt care after a twisting injury with rapid swelling.

At Aman Hospital, Dr. Gelber examines the meniscus, cartilage, kneecap and tendons and discusses your training. An MRI is arranged when the history and examination suggest a structural injury.

Frequently asked questions

Should I stretch or rest a stiff knee after football?

If it is muscle tightness, gentle stretching and light activity help. If the knee itself is swollen, reduce impact and let it settle first.

Are deep squats bad for my knees?

Not for healthy knees with good technique and gradual loading. With a meniscus or kneecap problem, deep loaded squats may need to be limited for a time.

Does a knee that swells after every game need an MRI?

Often yes, because repeated swelling suggests a problem inside the joint. The decision depends on the examination.
