Patient question
My knee and leg swelled up after a long flight. Should I worry?
Quick answer
Mild puffiness of both ankles after a long flight is common and settles within a day or two. One leg that becomes swollen, warm and painful in the calf after flying can be a deep vein thrombosis and needs same-day assessment. A knee that is already injured may also swell more, says Doha knee surgeon Dr. Pablo Gelber.
Why legs and knees swell when flying
Sitting still for many hours with the knees bent lets fluid pool in the lower legs. The cabin environment and not drinking enough add to it. For many expats in Qatar, flights home to Europe, South Asia, the Americas or Australia last many hours, often with a connection, and it is normal to land with puffy ankles and a stiff knee.
If you already have a knee problem, such as arthritis, a recent injury or recent surgery, the joint itself can also become more swollen and stiff after hours in a cramped seat, followed by walking long airport distances with luggage.
Signs that need urgent care
- Swelling of one calf or the whole of one leg, rather than both ankles
- Calf pain, tenderness, warmth or redness, especially when walking or flexing the foot
- Sudden shortness of breath, chest pain or coughing blood, which can mean a clot has moved to the lungs and needs an ambulance
- A hot, red, swollen knee with fever
Risk of a clot is higher after recent knee surgery or a leg injury, in people with previous clots, during pregnancy, with some hormone treatments, and with cancer.
Flying more comfortably with a knee problem
- Choose an aisle seat so you can stretch and walk every hour or two
- Do ankle pumps and gentle knee bends in your seat regularly
- Drink water and limit alcohol and very salty food
- Consider below-knee compression stockings; ask your doctor if they suit you
- After recent knee surgery or injury, ask your surgeon when it is safe to fly and whether you need clot prevention
- On arrival, walk, then raise the leg and ice the knee if it is swollen
When to see a knee surgeon in Doha
If calf and leg swelling settle within a day or two but the knee itself stays swollen, stiff or painful, it is worth having it examined. Travel often makes an existing problem, such as a meniscus tear or arthritis, more noticeable rather than causing a new one. At Aman Hospital, Dr. Gelber can assess the knee and, if you have travel planned after knee surgery, advise on timing and precautions.
Frequently asked questions
How soon after knee surgery can I fly home?
It depends on the operation and your personal clot risk. Short flights are often possible within days of keyhole surgery, while long-haul flights after bigger operations are usually delayed. Your surgeon should advise you individually.
Are compression stockings enough to prevent a clot?
They help reduce swelling and lower risk for many travellers, but people at higher risk may need additional prevention. Discuss it with your doctor before a long flight.
