Patient question

Knee pain at night with weight loss or fever: when is it urgent?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

Knee pain at night that is constant, not eased by rest or changing position, and comes with fever, night sweats or unexplained weight loss needs prompt medical assessment. Most causes are treatable, but infection and, rarely, a bone tumour must be ruled out quickly, advises Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha.

Why these signs change things

Most night knee pain comes from arthritis, a meniscus tear or soft tissue irritation, and it varies with position and activity. Pain that is present whatever you do, builds over weeks, and comes with symptoms affecting the whole body is a different pattern. It suggests a process in the bone or joint that is not simply mechanical, and it should not be left to see if it settles.

Red flags to act on

  • Fever, chills or feeling generally unwell, especially with a hot, swollen knee
  • Night sweats that soak your clothes or bedding
  • Weight loss you cannot explain
  • Pain that is constant, deep and getting steadily worse over weeks
  • A lump or swelling near the knee that is growing
  • A history of cancer, a weakened immune system, recent surgery or an injection in the knee
  • In children and teenagers: night pain in one leg, a limp, or pain that wakes them repeatedly

What could be behind it

  • Septic arthritis or bone infection: usually a hot, swollen, very painful knee with fever, and an emergency
  • Inflammatory arthritis, gout or pseudogout, which can cause fever and severe night pain
  • Stress fractures or bone weakening, sometimes linked to osteoporosis
  • Bone tumours, either benign or, rarely, malignant, and occasionally cancer that has spread to bone from elsewhere

This list is not meant to alarm. Most people with knee pain at night have none of these. The point is that the red flags above make it important to check quickly.

What to do

If the knee is hot, red and swollen with fever, go to an emergency department in Qatar the same day. For constant night pain with weight loss or night sweats but no fever, see a doctor within days rather than weeks.

Assessment usually includes an examination, blood tests, an X-ray and often an MRI. At Aman Hospital in Doha, Dr. Gelber arranges these promptly and coordinates with other specialists when the cause lies outside the knee.

Frequently asked questions

Does knee pain at night mean I have cancer?

Very rarely. Bone tumours are uncommon causes of knee pain. But persistent night pain with weight loss, sweats or a growing lump should always be investigated, so that a serious cause is not missed.

Could it be an infection if I have not had surgery?

Yes. Joint infection can occur without surgery, for example spreading through the bloodstream, particularly in people with diabetes, a weakened immune system or a skin wound near the knee.
