Patient question

Why is the inside of my knee swollen and sore after exercise?

Quick answer

When the inner knee hurts and the whole knee puffs up after exercise, the swelling is a sign that something inside the joint is irritated, most often a medial meniscus tear, cartilage damage or early arthritis. It deserves an examination rather than repeated ice, advises Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha.

What the swelling tells you

A healthy knee does not swell after normal exercise. When the lining of the joint is irritated by a torn flap of meniscus, a loose piece of cartilage or worn surfaces, it produces extra fluid. The knee looks puffy, the hollows beside the kneecap fill in, and full bending feels tight.

Swelling that appears the evening after exercise, or the next morning, and settles over a day or two, is a typical pattern of a meniscus or cartilage problem. Swelling within an hour or two of an injury suggests bleeding into the joint, which is more often a ligament tear or fracture.

Likely causes with pain on the inner side

  • Medial meniscus tear, from a twist or developing gradually after 40
  • Cartilage damage or an osteochondral lesion on the inner thigh bone
  • Early inner-knee osteoarthritis, with swelling after busy days
  • MCL sprain combined with a joint injury
  • Gout or other inflammatory arthritis, typically with heat, redness and severe pain

What to do now

Reduce the activity that triggers swelling, especially running, jumping, deep squats and pivoting sports, for a couple of weeks. Cycling and swimming are usually tolerated. A cold pack, compression sleeve and elevation help in the first days after a flare.

Keep the knee moving and keep the thigh muscle working with simple exercises such as straight-leg raises, because swelling quickly switches off the quadriceps. If swelling keeps returning each time you go back to sport, this is the signal to get the knee examined rather than repeating the same cycle.

When to see a knee surgeon in Doha

Seek urgent care if the knee is hot, red and very painful, or you have fever. Get prompt assessment if the knee swelled quickly after an injury, locks, or gives way. Book an appointment if swelling comes back after exercise for more than three or four weeks.

At Aman Hospital, Dr. Gelber examines the joint, may use ultrasound to assess the fluid, and arranges an MRI to look at the meniscus and cartilage when it will change treatment. Fluid is drawn off and tested when infection or crystals are a concern.

Frequently asked questions

Is it safe to keep exercising with a swollen knee?

Low-impact activity that does not increase swelling is usually fine. Pushing through repeated swelling with impact sport can worsen a meniscus or cartilage problem.

Will draining the fluid fix it?

Drawing off fluid can relieve pressure and help with diagnosis, but the swelling usually returns unless the underlying cause is treated.
