Patient question

Should ACL surgery be done early or can it wait?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

What matters most is operating on a calm knee: swelling down, full straightening and good thigh control, which often takes a few weeks. Waiting several months is reasonable if the knee stays stable, but repeated giving way risks new meniscus and cartilage damage, explains Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha.

Why the knee should be ready first

Operating on a stiff, swollen knee straight after injury can make stiffness after surgery more likely. A short spell of prehabilitation to reduce swelling, regain full extension and wake up the quadriceps usually leads to an easier recovery.

The old idea of always waiting a set number of weeks has given way to a simpler rule: operate when the knee is ready, not by the calendar.

Earlier and delayed surgery side by side

  • Earlier (once the knee is calm, usually within weeks to a few months): fewer chances for further giving way; meniscus tears more often repairable; quicker overall return for athletes
  • Delayed (many months or longer): time to try physiotherapy alone and see if it is enough; fits around exams, travel or work
  • Risk of waiting with an unstable knee: new meniscus tears and cartilage wear with each slip
  • Risk of rushing a swollen, stiff knee: harder rehabilitation and more stiffness
  • Needs urgent attention instead: a knee locked by a displaced meniscus tear, or several ligaments torn together

Planning around life in Qatar

Many patients time surgery around the football season, school holidays, Ramadan or the hottest summer months, when rehabilitation outdoors is harder. That is reasonable if the knee is not giving way in the meantime. Keep up physiotherapy while you wait, avoid pivoting sports, and report any new slipping or locking.

When to see a knee surgeon in Doha

Book an assessment within the first few weeks after an ACL injury, even if you are unsure about surgery, so meniscus damage is not missed. At Aman Hospital, Dr. Gelber checks whether the knee is ready, reviews the MRI and helps you choose a date that suits both the knee and your plans.

Frequently asked questions

Is it too late to have ACL surgery after a year?

No. ACL reconstruction can be done years later, though by then there is more often meniscus or cartilage damage to deal with as well.

Why does my surgeon want physiotherapy before the operation?

A knee that straightens fully and has active thigh muscles before surgery usually recovers more smoothly after it.
