Patient question

ACL surgery or physiotherapy only: which is right for my torn ACL?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

Physiotherapy alone can work well for people who avoid pivoting sports and whose knee stops giving way after rehabilitation. Reconstruction usually suits those returning to football, padel or basketball, manual workers on uneven ground, and anyone with repeated giving way or a repairable meniscus tear, explains Dr. Pablo Gelber, Doha knee surgeon at Aman Hospital.

Both are real treatment choices

A torn ACL does not always need an operation. Strong thigh and hip muscles and good balance can control many knees during straight-line activities such as walking, cycling, swimming and gym work. Rehabilitation alone is a legitimate first choice, not a lesser one.

The question is whether your knee stays stable for the life you want. That is often clear after a few months of structured physiotherapy.

Physiotherapy only and surgery side by side

  • Tends to suit physio only: walking, running in straight lines, cycling, swimming, lower-demand work, people who accept changing sport
  • Tends to suit surgery: football, padel, basketball, martial arts, jobs with climbing or uneven ground, young athletes
  • Recovery time: physio alone often a few months of focused work; surgery usually 9 to 12 months before pivoting sport
  • Main risk of physio only: further giving way that can damage the meniscus and cartilage
  • Main risks of surgery: stiffness, graft re-tear, the general risks of an operation
  • Flexibility: starting with physio does not rule out reconstruction later if the knee proves unstable

Signs that physiotherapy alone is not enough

Rethink the plan if the knee gives way during daily life, swells after each episode, or you develop new catching or locking. Repeated slips are the main reason ACL-deficient knees later develop meniscus tears. A locked knee or a large repairable meniscus tear is often a reason to operate sooner.

When to see a knee surgeon in Doha

See a knee surgeon early, ideally within a few weeks of the injury, so the decision is made with the full picture rather than after further damage. At Aman Hospital, Dr. Gelber examines stability, reviews the MRI for meniscus and cartilage injury and talks through your sport, work and goals before recommending either path.

Frequently asked questions

Can I go back to football without an ACL?

Some people try, but many knees give way with cutting and turning. Those returning to pivoting sports are usually advised to consider reconstruction.

Am I too old for ACL surgery?

Age alone rarely decides. Activity level, knee stability and the state of the cartilage matter more than the number of birthdays.
