Patient question
Both my knees crack when I stand up from prayer. Is that normal?
Quick answer
Cracking in both knees as you rise from sujood or from sitting on your heels is very common and usually harmless when there is no pain or swelling. Deep bending followed by a quick straightening releases gas and moves tissues around the kneecap, explains Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha.
Why prayer positions make knees crack
Sitting back on the heels bends the knee almost fully, and kneeling presses the kneecap against the floor. When you then push up to standing, the joint opens and the kneecap slides quickly back along its groove. That change in pressure can release small gas bubbles in the joint fluid and make tendons snap lightly over bone.
Because it happens in both knees, at the same moment, several times a day, and does not hurt, the pattern itself is reassuring. Problems inside one knee tend to affect that knee, not both equally.
When it is worth a closer look
- One knee is painful, swollen or stiff while the other is just noisy
- Aching at the front of the knee in sujood or on stairs
- Pain deep in the back of the knee when sitting on the heels
- A soft lump over the kneecap after frequent kneeling
- Morning stiffness lasting more than half an hour, which can come with arthritis
Making rising easier
Rising slowly and using your hands on the thighs or the floor spreads the load. A thicker prayer mat or cushion reduces pressure on the kneecaps. Simple strengthening, such as sit-to-stand from a chair, bridges and straight leg raises, makes standing up from the floor easier and often quieter.
If the knees start to hurt in these positions, some patients use a low stool or pray seated on a chair for a period while the knee settles. Patients can discuss the religious ruling with a scholar; the medical aim is simply to reduce strain while the knee recovers.
When to see a knee surgeon in Doha
Book an assessment if the cracking comes with pain, swelling, catching or difficulty getting up from the floor that is getting worse. At Aman Hospital, Dr. Gelber examines both knees, checks the kneecaps and joint lines, and orders an X-ray only when arthritis or another structural cause is suspected.
Frequently asked questions
Is the cracking damaging my knees over time?
Painless cracking is not thought to damage the joint or cause arthritis. Regular movement in prayer is part of keeping knees mobile.
My knees crack more during Ramadan. Why?
Longer night prayers mean more repetitions of kneeling and rising, often with less sleep and changes in routine. More cycles simply mean more chances to hear the noise. Pain or swelling is what matters.
