Patient question

Can an ACL tear be missed on an X-ray or MRI?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

An X-ray cannot show the ACL itself, so a normal X-ray does not exclude a tear. MRI detects most ACL tears, but early swelling, partial tears or an old injury can make reports unclear, so examination findings carry real weight, explains Dr. Pablo Gelber, knee surgeon at Aman Hospital in Doha.

What an X-ray can and cannot show

X-rays show bone, not ligaments. After a twisting injury they are useful to look for a fracture, a small flake of bone pulled off by a ligament, or signs of arthritis. In children and teenagers they also show the growth plates.

One small fracture on the outer edge of the shin bone, called a Segond fracture, strongly suggests an ACL tear. Usually, though, the X-ray after an ACL tear looks normal, which is why people are sometimes told nothing is broken and sent home.

How reliable MRI is for the ACL

MRI is the standard scan for a suspected ACL tear and picks up most complete tears. It also shows bone bruising, which follows a typical pattern when the knee has shifted, and any meniscus, cartilage or other ligament injury.

Reports can be less clear in some situations:

  • A scan done in the first days, when bleeding and swelling blur the picture
  • A partial tear, where some fibres remain
  • An old tear, where the ligament has scarred onto a neighbouring structure and can look continuous
  • Scan quality, or knee position during the scan

Why the examination still matters

The Lachman test checks how far the shin bone moves forward, and the pivot-shift test reproduces the giving-way movement. When the examination shows clear looseness, that counts even if the MRI report sounds reassuring. When the examination is stable, a worrying report may matter less.

Treatment decisions are based on how the knee behaves, not on the scan wording alone.

Bringing your scans to Aman Hospital

If you already have an MRI from another clinic in Qatar or from home, bring the images, not only the report. Dr. Gelber reviews the images himself alongside the examination. A repeat scan is only arranged if it would change the plan, for example if the first was done very early or the quality is poor.

Frequently asked questions

My MRI says ACL sprain. Is that the same as a tear?

Sprain is a broad word that can mean stretched fibres, a partial tear or sometimes more. The examination and the images themselves clarify how much of the ligament is working.

Is a CT scan better than MRI for an ACL tear?

Not for diagnosing the tear. CT is mainly used to look at bone, for example when planning revision surgery or assessing a fracture.

Can arthroscopy be used just to diagnose an ACL tear?

Rarely. Examination and MRI usually give the answer. Arthroscopy is normally done as part of treatment rather than purely as a test.
