Patient question

Am I too old for ACL reconstruction?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

There is no fixed upper age limit for ACL reconstruction. Active people in their forties, fifties and beyond can benefit if the knee keeps giving way during the sports they enjoy. The state of the cartilage and your goals matter more than age, explains Dr. Pablo Gelber, who assesses ACL tears at Aman Hospital in Doha.

Why age alone does not decide

Many adults in Qatar stay active well into middle age, playing padel, football, tennis, skiing on holiday or training in the gym. For them, a knee that gives way can limit both sport and daily confidence. Studies of older patients have shown that reconstruction can restore stability and function in carefully selected people.

On the other hand, many older adults with an ACL tear do well without surgery, especially if their activities do not involve pivoting.

What matters more than age

  • How often the knee gives way, and during which activities
  • The sports or work you want to return to
  • Existing arthritis: significant wear may mean a different operation, such as an osteotomy, is more useful, or that reconstruction will not relieve pain
  • Meniscus damage and whether it can be repaired
  • General health, weight and willingness to commit to months of rehabilitation
  • Bone quality and any previous surgery

Recovery in older patients

Recovery follows the same broad stages, though some people take longer to rebuild muscle strength. Stiffness can be more of a concern, so early movement and consistent physiotherapy matter. Goals are often practical: confident padel or tennis, hiking, playing with grandchildren, or working without the knee giving way.

Making the decision at Aman Hospital

Dr. Gelber examines the knee, reviews MRI and standing X-rays to assess the cartilage and alignment, and discusses what you want your knee to do. You may be advised to try a structured rehabilitation programme first, with surgery reserved for ongoing instability.

Frequently asked questions

Is the operation riskier after 50?

General surgical risks rise somewhat with age and medical conditions, and stiffness may be more common. These are discussed individually before deciding.

If I have arthritis, will ACL surgery help my pain?

ACL reconstruction treats instability, not arthritis pain. If pain from wear is the main problem, other treatments may be more suitable.
