Patient question

How urgent is surgery for a bucket-handle tear? Can I wait?

Dr. Pablo GelberOrthopaedic Knee Surgeon

Quick answer

A locked knee from a bucket-handle tear is rarely a same-night emergency, but it is urgent: assessment within days and surgery within a few weeks is generally advised. Delay can damage the torn strip and make repair less likely, explains Dr. Pablo Gelber, knee surgeon at Aman Hospital, Doha.

Why timing matters

While the strip is flipped into the middle of the knee, it is pinched every time you walk. Over weeks it can fray, shrink or tear further, so a strip that could have been stitched back may have to be trimmed away instead. A knee that stays bent for a long time can also become stiff and the thigh muscle weakens quickly.

In younger patients, fresh tears in good-quality tissue tend to give repair the strongest chance of healing.

When to go to the emergency department

  • Very rapid, tense swelling within an hour or two
  • A deformed knee or suspicion of a fracture or dislocation
  • Numbness, coldness or pale colour in the foot
  • Severe pain that simple pain relief does not touch
  • A hot, red knee with fever

When planning around life is reasonable

If the knee unlocks and you can walk, a short delay of days to a couple of weeks to arrange work, family or Ramadan commitments is usually acceptable after you have been assessed. Planning a long-haul flight home with a locked knee is less ideal, as sitting cramped for hours is uncomfortable and raises the risk of blood clots after injury. Discuss travel plans with your surgeon before booking.

What Dr. Gelber will do

At Aman Hospital in Doha, the first visit confirms whether the knee is locked, checks the ACL and other ligaments and arranges an MRI if not already done. If surgery is needed, he explains whether repair looks likely, what to expect afterwards and how to schedule it around your commitments in Qatar.

Frequently asked questions

Is it ever too late to repair a bucket-handle tear?

Older tears can sometimes still be repaired, but the chance generally falls as the strip becomes damaged. The decision is made on the tissue quality seen during arthroscopy.

Can I fast during Ramadan before surgery?

Many patients do. Surgery timing and fasting around the operation should be agreed with your surgeon and anaesthetist.
