Quadriceps muscle (quads)
Quick answer
The quadriceps, or quads, are the group of four muscles at the front of the thigh that straighten the knee and control it as you sit, squat, land and go downstairs. Their strength is central to nearly every knee recovery, notes Doha knee surgeon Dr. Pablo Gelber, who treats patients at Aman Hospital.
What it is
The four parts are the rectus femoris, which also crosses the hip, and the vastus lateralis, vastus medialis and vastus intermedius. They join above the knee into the quadriceps tendon, which surrounds the kneecap and continues as the patellar tendon to the tibia. The lower, teardrop-shaped part of the vastus medialis, often called the VMO, sits just above and inside the kneecap.
Why it matters for patients
Strong quads act as shock absorbers and protect the joint, cartilage and ligaments. After any knee injury, surgery or swelling, the brain partly switches off the quadriceps, and the thigh can visibly shrink within days. Rebuilding that strength is a main goal of physiotherapy after ACL reconstruction, meniscus surgery and knee replacement.
Weak quads are linked with front knee pain, a feeling of the knee giving way and difficulty on stairs or getting up from the floor. In knee osteoarthritis, quadriceps strengthening is one of the most effective non-surgical treatments.
How it is assessed
A physiotherapist or surgeon looks for muscle wasting, measures thigh size and checks whether you can fully lock the knee straight. Strength testing on machines helps guide return to sport.
Frequently asked questions
Why does my thigh look smaller after knee surgery?
Pain and swelling switch off the quadriceps and the muscle quickly loses bulk. With a structured exercise programme it usually returns over the following months.
What is a simple quad exercise I can do at home?
Static quad sets, tightening the thigh to press the back of the knee down while lying with the leg straight, are a common starting point. A physiotherapist can tailor a programme to your knee.
